Effect of General Anesthesia Combined with Transverse Abdominal Muscle Block on Stress Response in Patients with Lower Abdominal Surgery

摘要 目的:探讨全身麻醉联合腹横肌平面阻滞对下腹部手术患者应激反应和过敏的影响。方法:选择2016年1月~2017年7月80例下腹部手术患者进行分组,对照组实施全身麻醉,观察组实施全身麻醉联合腹横肌平面阻滞,比较两组术后2h和12h下腹部疼痛程度、不同时间点患者机体应激指标血糖和皮质醇水平。结果:观察组下腹部手术麻醉总有效率高于对照组,P<0.05;切皮前、切皮后1h两组机体应激指标血糖和皮质醇水平相近,P>0.05;术后6h观察组机体应激指标血糖和皮质醇水平低于对照组,P<0.05;观察组过敏反应发生率低于对照组,P<0.05。结论:全身麻醉联合腹横肌平面阻滞,可显著降低术后6h机体应激指标血糖和皮质醇水平,减少过敏应激,值得推广和应用。 Objective: To explore the effect of general anesthesia combined with transverse abdominal muscles block on the stress response and allergy in patients with lower abdominal surgery. Methods: Patients with abdominal operation from January 2016 to July 2017 were selected and grouped. The control group received general anesthesia, and the observation group received general anesthesia combined with transverse abdominal muscles block. The degree of abdominal pain after 2 h and 12 h, indicators of blood sugar with stress response and cortisol levels at different points of time was compared between the two groups. Results: The total effective rate of anesthesia was higher in the observation group than in the control group, P<0.05. The blood glucose and cortisol levels of the two groups were similar before and one hour after cutting and peeling, P>0.05. Blood glucose and cortisol levels of the body stress indicators were lower in the observation group 6 hours after operation than in the control group, P<0.05. The incidence of anaphylaxis in the observation group was lower than that in the control group, P<0.05. Conclusion: General anesthesia combined with transverse abdominal muscles block can significantly reduce the blood glucose and cortisol levels in the 6 hours after surgery,and reduce the allergic stress,which is worthy of promotion and application.
