摘要
目的 临床认为心力衰竭与相关治疗均可致使其电解质水平变化,而鲜见急性心力衰竭高血压患者电解质变化的相关临床研究。本研究探讨血清电解质水平检测对高血压患者急性心力衰竭后病情评估的应用价值。方法 收集2015-03-01-2018-10-01郑州市第九人民医院收治的118例高血压伴急性心力衰竭患者作为病例组,将同院同期接受健康查体的25名健康志愿者作为对照组。所有入选者均需接受血清电解质水平[氯离子(CI-)、钙离子(Ca2+)、钾离子(K+)、钠离子(Na+)]检查,分析检查结果。结果 病例组CI-、Ca2+、K+和Na+水平均低于对照组,差异有统计学意义,均P<0.05。正常血清CI-(Z=44.207,P<0.001)、Ca2+(Z=77.640,P<0.001)、K+(Z=10.470,P=0.005)和Na+(Z=34.633,P<0.001)水平患者中,重度高血压占比均低于低、高电解质水平患者,差异有统计学意义。正常血清CI-(Z=50.568,P<0.001)、Ca2+(Z=12.382,P=0.002)、K+(Z=8.143,P=0.017)和Na+(Z=46.577,P<0.001)水平患者中,KillipⅣ级占比均低于低、高电解质水平患者,差异有统计学意义。结论 血清电解质水平可对高血压严重程度以及急性心力衰竭严重程度给予评估,以利于临床及时采取干预措施。
OBJECTIVE To investigate the application value of serum electrolyte level detection in the assessment of patients with hypertension combined with acute heart failure.METHODS A total of 118hypertensive patients with acute heart failure admitted to the Ninth People′s Hospital of Zhengzhou City from March 1,2015to October 31,2018were selected as case group,and 25healthy volunteers who received health examination in the same hospital at the same time were selected as control group.All the participants were required to detect serum electrolyte levels[chloride ion(CI-),calcium ion(Ca2+),potassium ion(K+),sodium ion(Na+)]and analyze the results.RESULTS The levels of CI-,Ca2+,K+,Na+in the case group were lower than those in the control group,with statistical significance(all P<0.05).The proportion of moderate and severe hypertension in patients with normal serum CI-(Z=44.207,P<0.001),Ca2+(Z=77.640,P<0.001),K+(Z=10.470,P=0.005),Na+(Z=34.633,P<0.001)levels was lower than that in patients with low and high electrolyte levels,and the difference was significant.The percentage of KillipⅣin patients with normal serum CI-(Z=50.568,P<0.001),Ca2+(Z=12.382,P=0.002),K+(Z=8.143,P=0.017),Na+(Z=46.577,P<0.001)levels was lower than that in patients with low and high electrolyte levels.The difference was statistically significant.CONCLUSION Serum electrolyte level can evaluate the severity of hypertension and acute heart failure,so as to facilitate timely intervention in clinic.
