短节段股骨近端锁定钢板联合缝匠肌髂骨瓣治疗青年型股骨颈骨折26例近期临床观察

Short segmental femoral proximal locking plate coupled with sartorius ilium flap in treatment of femoral neck fractures in young adults with 26 cases of recent clinical observation

目的探讨短节段股骨近端锁定钢板联合缝匠肌骨瓣治疗青年型股骨颈头下型骨折的可行性、安全性及有效性。方法回顾分析2008年01月~2012年12月期间收治的26例青年股骨颈骨折行短节段股骨近端锁定钢板联合缝匠肌骨瓣治疗,男18例,女8例,年龄21~42岁,平均35.4岁,按照Garden分型系统,GardenⅠ型2例,GardenⅡ型7例,GardenⅢ型11例,GardenⅣ型6例。患者分别于术后3、6、9、12月复查髋关节CT,分析骨折端的愈合程度。结果 26例手术全部成功,术后8~10个月复查CT提示骨折均愈合,尚无股骨头缺血性坏死病例。

Objective To investigate the feasibility, safety and availablity of using short segmental femoral proximal locking plate coupled with sartorius ilium flap in the treatment of the femoral neck fractures in yonug adults. Methods From January2008 to December 2012, a total of 26 young patients were treated with short segmental femoral proximal locking plate coupled with sartorius ilium flap at our institutions. The clinical results were reviewed retrospectively. 18 were males,and 8 females. The average age of the patients at the times of injury was 35.4 years(range, 21 to 42 years). According to garden classification for femoral neck fractures, 2 cases were graded as Ⅰ, 7 Ⅱ, 1 Ⅲ, and 6 Ⅳ. To evaluate the progress of bone healing, the spiral CT scanning of hip was done at 3, 6, 9and 12 months postoperative. Results Surgery was successfully for all patients. Follow-ups of 8 to 10 months showed that fractures healed was achieved in all the cases according to the observation of postoperative CT examination, and no patient demonstrated osteonecrosis of the humeral head. Conclusion Short segmental femoral proximal locking plate coupled with sartorius ilium flap? clinical curative effect in treatment of femoral neck fractures in young adults is satisfied.
