抗-Le^b导致ABO血型正反定型不符分析

Analysis of positive and negative typing of ABO blood group caused by anti-Le^b

目的:回顾性分析3例由抗-Leb导致的ABO血型正反定型不符的血清学特点,为抗-Leb干扰的血型鉴定和配血困难提供解决方案。方法:血型鉴定采用强生AutoVUE玻璃珠卡式法,正反定型不一致样本用试管法进行复查。抗体筛查强生AutoVUE玻璃珠卡式进行检测,阳性标本用Dinna抗人球卡作抗体鉴定,必要时加做盐水试管法。试管法鉴定被检者红细胞上是否存在-Leb抗原,交叉配血由强生AutoVUE抗人球卡和盐水法进行检测。结果:①3例抗筛阳性标本均因抗-Leb抗体影响ABO反定型,用3人份Leb-红细胞辅助鉴定,可排除抗-Leb抗体干扰;②抗体鉴定结果均为抗-Leb;③用抗原分型方法作交叉配血配血均相合。结论:抗-Leb引起正反定型不一致时,应使用-Leb抗原阴性红细胞进行血型反定型,同时应选用-Leb阴性的供者配血以确保结果的准确性。

Objective::To retrospectively analyze the serological characteristics of 3 cases of positive and negative typing of ABO blood group caused by anti-Le^b,in order to provide a solution for the difficulty of blood type identification and blood matching caused by anti-Le^b.Method:Johnson AutoVUE glass bead cassette method was used to identify blood group,and the positive and negative inconsistency samples were reexamined by test tube method.The positive samples were identified by Dinna anti-human globulin card and,if necessary,by salt water test tube method.The test tube method was used to identify the presence of-Le^b antigen on the erythrocytes of the tested persons.Johnson AutoVUE anti-human globulin card and brine method were detected.Result:①Three anti-sieve positive specimens were all affected by anti-Le^b antibody to reverse typing of ABO.The anti-Le^bantibody interference could be eliminated by using 3 human Le^b-red blood cells to assist identification.②The results of antibody identification were all anti-Le^b.③The antigenic typing method was used as cross matching homozygous.Conclusion:When anti-Le^bcauses inconsistency between positive and negative typing,negative RBC with Le^b antigen should be used for blood type reverse typing,and donors with negative-Le^b should be selected to match blood to ensure the accuracy of the results.
