小儿重症支原体肺炎阿奇霉素干预疗效及对患者免疫功能、肺功能的变化影响及其相互关系 被引量:25

摘要 目的:探究小儿重症支原体肺炎阿奇霉素干预的疗效及免疫功能和肺功能的变化及其相互关系。方法:选取重症支原体肺炎患儿128例,将其随机分为观察组和对照组各64例。对照组给予红霉素治疗,观察组则以阿奇霉素进行干预,比较两组患儿的疗效以及观察组中治疗总效率不同患儿之间的免疫功能指标(IgA、IgG、IgM)和肺功能指标(FEV1、FVC、PEF、MEF50)的变化及其相互关系。结果:观察组患儿治疗的总有效率87.50%,明显高于对照组的治疗总有效率71.87%(P<0.05)。观察组无效组IgA、IgG、IgM明显低于有效组,而FEV1、FVC、PEF、MEF50则明显高于有效组(P<0.05)。Pearson分析结果显示,IgA、IgG、IgM与其FEV1、 FVC、PEF、MEF50呈负相关关系(P均<0.05)。结论:小儿重症支原体肺炎阿奇霉素干预的疗效显著,患儿的免疫功能指标和肺功能指标均明显改善,且其免疫功能和肺功能呈负相关关系。 Objective: To study the effects of azithromycin intervention on severe mycoplasma pneumoniae in children and changes in immune function and lung function and their correlation.Methods: 128 patients children treated in our hospital from March, 2017 to May, 2018 were randomly divided into observation group(n=64) and control group(n=64). The control group was treated with erythromycin and the observation group was treated with azithromycin. The efficacy of the two groups was compared,The changes of immunological function indicators(IgA, IgG, IgM) and pulmonary function indicators(FEV1, FVC, PEF, MEF50) of the observation group with different overall therapeutic efficiency were compared and their correlation was analyzed. Result: The total effective rate of treatment in the observation group was 87.50%, which was significantly higher than the control group(71.87%)(P<0.05). In the observation group,the patients of significant effect and effect were regarded as the effective group, invalid cases were regarded as invalid groups. The IgA, IgG and IgM of the invalid group were significantly lower than that of the effective group, while FEV1, FVC, PEF and MEF50 were significantly higher than those of the effective group(P<0.05). Pearson analysis showed that IgA, IgG and IgM were negatively correlated with FEV1, FVC, PEF and MEF50(P all <0.05). Conclusion: The efficacy of azithromycin intervention in severe mycoplasma pneumonia in children has significant, the immune function and lung function of children have significantly improved, and have negatively correlated.
