肌层浸润性膀胱癌患者保留膀胱术后的预后状况研究

Study of Prognosis of Partial Cystectomy for Muscle-invasive Bladder Cancer Patients

目的探究肌层浸润性膀胱癌患者保留膀胱术后的预后状况及其影响因素。方法回顾性分析50例肌层浸润性膀胱癌患者的临床资料,均行膀胱部分切除术配合辅助化疗治疗,COX回归分析影响术后预后复发及生存的因素。结果 5年总生存率为60.00%,5年总复发率为46.00%;经单因素筛选及多因素回归分析后,淋巴血管侵犯、输尿管再植术等均是影响术后无复发率与总生存率的独立危险因素(P<0.05)。结论肌层浸润性膀胱癌患者保留膀胱术后的预后状况,与淋巴血管侵犯、输尿管再植术等因素密切相关,应引起临床重视。

Objective To investigate the prognosis and its influence factors of partial cystectomy for muscle -invasive bladder cancer patients .Methods The clinical data of 50 patients with muscle-invasive bladder cancer were retrospectively ana-lyzed,all of them were treated by partial cystectomy and adjuvant chemotherapy treatment ,and the influence factors of postopera-tive recurrence and survival were analyzed by COX regression analysis .Results The 5-year total survival rate was 60.00%, while the 5-year total recurrence rate was 46.00%.After Logistic single factor screening and multi-factor analysis,lymphovascular invasion and ureteral reimplantation were the independent risk factors of postoperative recurrence and overall survival (P<0.05). Conclusion The prognosis of partial cystectomy for muscle-invasive bladder cancer patients is related to the lymphovascular in-vasion and ureteral reimplantation closely ,which should be paid attention during clinical .
