诱导化疗NSCLC患者肿瘤体积变化状况及危险因素研究

Study on Quantification of Tumor Size Changes and Its Risk Factors after Induction Chemotherapy for Non-small Cell Lung Cancer

目的探讨诱导化疗非小细胞肺癌(NSCLC)患者肿瘤体积变化状况及危险因素。方法回顾性分析于我院进行化疗的NSCLC患者120例;采用螺旋CT获取肿瘤的图像并采用相应的治疗计划系统(TPS)自带的体积测量软件进行肿瘤体积的测量,分析诱导化疗NSCLC患者肿瘤体积变化状况及影响化疗1周期之后体积变化的危险因素。结果诱导化疗NSCLC患者1周期后平均的原发灶的肿瘤体积比化疗之前明显下降(P<0.05),差异具有统计学意义;在化疗第2、3、4周期之后肿瘤的体积无明显变化,但第5周期后肿瘤的体积出现增大的现象,与第4周期相比(P<0.05),差异具有统计学意义;第6周期的肿瘤体积比第5周期体积无明显的变化。原始肿瘤体积大小和T分期是影响诱导化疗第1周期之后肿瘤的体积变化的危险因素,肿瘤的原始体积≥124 cm3或分期T1-2的患者在化疗1周期之后的肿块的缩小更明显。结论诱导化疗NSCLC,在1周期之后疗效可能会达到最佳,最好不要连续化疗超过4个周期;肿瘤体积和T分期是肿瘤体积变化的危险因素。

Objective To explore the quantification of tumor size changes and its risk factors after chemotherapy for non -small cell lung cancer .Methods 120 NSCLC patients who received chemotherapy were retrospectively analyzed .Spiral CT tumor images were acquired and corresponding treatment planning system ( TPS) was used to measure tumor size .And then the tumor size changes and risk factors of induction chemotherapy in NSCLC patients were analyzed after 1 cycles of chemotherapy .Results Af-ter 1 cycles of induction chemotherapy ,the average tumor size decreased significantly ( P<0.05) ,the difference was statistically significant.After 2,3 and 4 cycles of chemotherapy ,the tumor size had no significant change ,but compared with the 4th cycle,the tumor size increased in the 5th cycles(P<0.05),the difference was statistically significant;compared the 5th cycle,the tumor size in the 6th cycle had no obvious change .The original tumor size and T staging after 1 cycle of induction chemotherapy were the risk factor affecting change of tumor size .Patients with original tumor size ≥124 cm3 or stage T1-2 decreased more obviously after 1 cycles of chemotherapy .Conclusion Induction chemotherapy for NSCLC may achieve the best curative effect after 1 cycle.The best cycles should be no more than 4 cycles.The tumor size and T staging are the risk factors for tumor size change .
