rhG-CSF在急性淋巴细胞白血病患儿CODP方案化疗中的辅助作用

Assistant Role of rhG-CSF in CODP Chemotherapy for Children with Acute Lymphoblastic Leukemia

目的探讨基因重组人粒细胞集落刺激因子(rhG-CSF)在急性淋巴细胞白血病患儿CODP方案化疗中的辅助作用。方法选取急性淋巴细胞白血病化疗患儿80例,根据不同治疗方法将患者分为观察组和对照组,各40例。对照组给予CODP(柔红霉素、长春新碱、环磷酰胺、泼尼松)方案化疗,观察组在对照组基础上给予rhG-CSF辅助化疗,比较2组患儿化疗效果。结果观察组完全缓解率为100.0%,对照组完全缓解率92.5%、部分缓解率7.5%,2组患儿化疗总有效率比较差异无统计学意义(P>0.05)。观察组骨髓抑制(白细胞计数、中性粒细胞和血小板计数下降)和口腔感染发生率明显低于对照组,中性粒细胞恢复时间、白细胞恢复时间和发热持续时间亦明显少于对照组,2组比较差异具有统计学意义(P<0.05)。结论 rhG-CSF辅助急性淋巴细胞白血病CODP方案化疗对改善化疗引起的骨髓抑制、降低感染具有重要的临床意义。

Objective To investigate the auxiliary effect of recombinant human granulocyte colony stimulating factor ( rhG-CSF) in CODP chemotherapy for children with acute lymphoblastic leukemia .Methods 80 children with acute lympho-blastic leukemia treated with chemotherapy were divided into the observation group and the control group according to different treatment methods,each with 40 cases.The control group was given CODP (daunorubicin,vincristine,cyclophosphamide,predni-son) chemotherapy ,and the observation group additionally received rhG-CSF adjuvant chemotherapy ,the effects of the 2 groups werecompared.Results Complete remission rate in the observation group was 100.0%,complete remission rate in the control group was 92.5%,and partial remission rate was 7.5%,there had no significant difference (P>0.05).Bone marrow depression (decreased white blood cell count ,neutrophil,platelet count drop) and oral infection rate in the observation group were signifi-cantly lower than those of the control group ,the neutrophil recovery time ,WBC recovery time and duration of fever in the observa-tion group were obviously less than those of the control group ,there had significant difference (P<0.05).Conclusion rhG-CSF CODP adjuvant chemotherapy for acute lymphoblastic leukemia has important clinical significance in improving bone marrow sup -pression and reducing infection .
