单操作孔胸腔镜下手术治疗早期非小细胞肺癌患者的效果研究

The Efficacy of Uniport Video-assisted Thoracoscopic Surgery for Patients with Early Stage Non-small Cell Lung Cancer

目的探讨通过单操作孔胸腔镜下手术(VATS)治疗早期非小细胞肺癌患者的效果。方法选取2011年1月至2014年5月行单操作孔胸腔镜下手术治疗的早期非小细胞肺癌患者110例(观察组),与同期胸腔镜辅助小切口(video-assisted mini-thoracotomy,VAMT)肺叶切除术的早期非小细胞肺癌患者110例(对照组),分别比较2组患者的手术时间、术中出血量、引流管留置时间、术后总引流量、淋巴结清扫数目、术后并发症、术后疼痛程度等。结果 2组患者均顺利完成手术,无死亡病例。2组患者的手术时间、淋巴结清扫数目以及术后并发症相比较,差异无统计学意义(P>0.05)。观察组患者出血量、术后总引流量、引流管放置天数和术后疼痛(术后第1~3天)评分均小于对照组(P<0.05)。结论单操作孔胸腔镜下手术治疗早期非小细胞肺癌疗效肯定,比VAMT创伤小,更易于恢复,值得在临床上推广应用。

Objective To explore the efficacy of uniport video-assisted thoracoscopic surgery( VATS) for patients with early stage non-small cell lung cancer. Methods 110 cases of non-small cell lung cancer treated with uniport VATS( observation group) and 110 cases of non-small cell lung cancer treated with video-assisted mini-thoracotomy( VAMT)( control group) were selected. The operative time,blood loss,drainage tube indwelling time,postoperative total drainage volume,number of lymph node dissection,postoperative complications,and the degree of postoperative pain were compared between the 2 groups. Results Both groups successfully completed the surgery with no deaths. There had no significant difference in operative time,number of lymph node dissection and postoperative complications between the 2 groups( P > 0. 05). The amount of bleeding,postoperative total drainage volume,drain time,and postoperative pain( 1 ~ 3 days after surgery) scores in the observation group were lower than those of the control group( P < 0. 05). Conclusion Uniport VATS for early stage non-small cell lung cancer is effective,has smaller trauma than VAMT,and it is conducive to recovery. It is worthy of clinical application.
