围手术期输血对胃癌根治手术患者并发症发生的影响

Effect of Perioperative Blood Transfusion on Postoperative Complications of Gastric Cancer after Radical Gastrectomy

目的探讨围手术期输血对行胃癌根治术的患者术后并发症的影响。方法选择236例胃癌患者,根据围手术期是否输血将其分为输血组87例(36.9%)和未输血组149例(63.1%),并系统分析围手术期输血与其临床病理参数的关系及其对术后并发症的影响。结果围手术期输血发生率与患者肿瘤大小、侵袭程度、淋巴结转移数、TNM分期及术前贫血紧密相关,但与患者性别、年龄及肿瘤位置无显著相关性(P>0.05)。输血组术后并发症发生率明显高于未输血组(P<0.01),并且输血量越多并发症发生率越高,但输血时间对其没有影响。结论围手术期输血与胃癌患者术后并发症增多有关,临床实践中应尽量减少输血,合理输血,降低术后并发症的发生率。

Objective To explore the effect of perioperative blood transfusion on postoperative complications of gastric cancer. Methods 236 patients with gastric cancer,according to whether received perioperative blood transfusion,were divided into the blood transfusion group( 87 cases,36. 9%) and without blood transfusion group( 149 cases,63. 1%),and systematic analysis of association between perioperative blood transfusion and clinicopathological parameters and its effects on postoperative complications were conducted. Results The incidence of perioperative blood transfusion were and closely related with tumor size,invasion,lymph node metastasis,TNM staging and preoperative anemia,but was not correlated with the gender,age and tumor location( P > 0. 05). The incidence of postoperative complications of blood transfusion group was obviously higher than that without blood transfusion group( P < 0. 01),and incidence of complications increased with the amount of blood transfusion,but blood transfusion time had no effect on the complications. Conclusion Perioperative blood transfusion is associated with with postoperative complications of gastric cancer patients,in clinical practice,blood transfusion should be reduced and used reasonably,in order to reduce the incidence of postoperative complications.
