血浆EBV-DNA表达水平与鼻咽癌分期的相关性分析

Correlation Analysis of Plasma EBV-DNA Expression and NPC Staging

目的研究血浆EBV-DNA表达水平与鼻咽癌分期的相关性。方法选择了55例鼻咽癌患者,同时选取同期住院治疗的55例其他类型的肿瘤患者作为其他类肿瘤对照组及55例体检健康者为健康对照组,比较分析鼻咽癌组与对照组患者的血浆EBV-DNA阳性检出情况,鼻咽癌组患者与对照组血浆EBV-DNA浓度及血浆EBV-DNA阳性的鼻咽患者早期和晚期的拷贝数。结果鼻咽癌组血浆EBV-DNA检出阳性率为72.7%,明显高于其他类肿瘤对照组的阳性率(23.7%)以及健康对照组(10.9%),具有统计学的意义(P<0.05)。鼻咽癌组患者的血浆EBV-DNA浓度较高,达到了13654.5 copies/m L,而其他类肿瘤对照组及健康对照组的血浆EBV-DNA浓度均为0,具有统计学的意义(P<0.05)。观察还发现血浆EBV-DNA阳性的鼻咽患者晚期血浆EBV-DNA拷贝数较高,达到了(201.48±176.19),明显高于早期的血浆EBV-DNA拷贝数(9.60±10.77),两组数据进行比较,具有统计学的意义(t=4.3312,P=0.0001)。结论荧光定量的PCR检测血浆EBV-DNA是1种比较可靠的方法,能够有效诊断鼻咽癌,且血浆EBV-DNA与鼻咽癌的分期关系密切,该方法适合广泛地推广。

Objective To research the correlation between the expression level of plasma EBV-DNA and nasopharyngeal carcinoma( NPC) staging. Methods 55 cases of NPC,55 cases of other types of cancer as tumor control group and 55 cases of healthy patients as healthy control group were selected,all patients were treated in our hospital at the same period. The positive rates and concentration of plasma EBV-DNA of the 3 groups,the plasma EBV-DNA copy number at early and last period of plasma EBV-DNA positive patients were compared and analyzed. Results The positive rate of NPC group was 72. 7%,which was significantly higher than of other types of tumors( 23. 7%) and healthy control group( 10. 9%),there had statistical difference( P< 0. 05). The concentrations of plasma EBV-DNA in NPC patients reached13654. 5 copies / m L,while that of other groups both were 0 copies / m L,there had statistical difference( P < 0. 05). Among the plasma EBV-DNA positive patients,the copies of plasma EBV-DNA at last period reached( 201. 48 ± 176. 19),which was higher than at early period( 9. 60 ± 10. 77),there had statistical difference( t = 4. 3312,P = 0. 0001). Conclusion Fluorescence quantitative PCR detection of plasma EBV-DNA is reliable,it can effectively diagnose NPC,and plasma EBV-DNA is closely related with NPC staging. It is worthy of promotion.
