质子泵抑制药物中长期使用对胃癌及癌前病变发病的影响

The Effect of Long-term Use of Proton Pump Inhibitors on the Incidence of Gastric Cancer and Precancerous Lesions

目的探讨中长期使用质子泵抑制剂对胃癌及癌前病变发病率的影响。方法选取82例患者作为研究对象,根据诊断情况和治疗方案将患者分为2组。采用前瞻性的研究方法观察中长期使用质子泵抑制剂对胃癌及癌前病变发病率的影响,比较2组患者研究前(0个月)和复查史的HP感染率变化,比较患者复查时的胃癌及癌前病变的发生率。结果 2组患者不同时间的HP感染率无统计学差异(P>0.05)。2组患者1年内的癌前病变发生率无统计学差异(P>0.05);24个月后,观察组癌前病变的发生率明显高于对照组(P<0.05)。2组患者胃癌的发生率无统计学差异(P>0.05)。结论尽管质子泵抑制类药物疗效显著,但长期的应用可能导致胃癌及癌前病变的发生率提高,故临床应用时应该充分考虑多种因素,避免药物滥用。

Objective To explore the effect of long-term use of proton pump inhibitors on the incidence of gastric carcinoma and precancerous lesions. Methods 82 cases of outpatients were selected as the research object,and according to the diagnosis result and therapy were divided into 2 groups. Advanced research methods was used to observe the effect of long-term use of proton pump inhibitor on the incidence of gastric cancer and precancerous lesions,changes of HP infection before the study( 0months) and during the review history were compared between the 2 groups,the incidence of gastric cancer and precancerous lesion were compared. Results The incidence of HP infection in different stages showed no significant difference between the 2groups( P > 0. 05),the incidence of precancerous lesions within 1 year of the 2 groups showed no significant difference( P >0. 05),24 months later,the incidence of precancerous lesion in the observation group was significantly higher than that of the control group( P < 0. 05). The incidence of gastric cancer of the 2 groups showed no significant difference( P > 0. 05). Conclusion Despite its significant effect,long-term use of proton pump inhibitors may lead to the increased incidence of gastric cancer and precancerous lesions,so in the clinical application various factors should be fully considered to avoid drug abuse.
