家庭肠内营养应用于胃癌晚期患者中的效果

Application Effect of Home Enteral Nutrition for Advanced Gastric Cancer

目的探讨家庭肠内应用对胃癌晚期患者营养状态以及生活质量的效果。方法选取96例晚期胃癌患者作为研究对象,其中46例患者采用家庭肠内营养支持为EN组,另50例患者进行院内肠外营养支持为PN组,比较两组患者营养支持前后营养状态的变化以及营养支持6个月后患者生活质量的变化。结果两组患者经营养支持治疗后其血液成分和机体组成成分均与营养支持前存在明显差异(P<0.05),营养支持6个月后两组患者在血液成分上并无明显差异(P>0.05),但EN组患者的体重、BMI以及脂肪群均明显高于PN组患者(P<0.05),且EN组患者KPS、OLI以及EORTC QLQ-C30功能评分均明显高于PN组,而EORTC QLQ-C30单项评分则明显低于PN组(P<0.05),差异均具有统计学意义。结论家庭肠内营养可有效改善胃癌晚期患者的营养状态,且相比院内肠外营养其对提高患者的生活质量效果更佳,可在具有适应症的胃癌晚期患者中推广应用。

Objective To study the effect of home enteral nutritional for nourishment state and life quality of advanced gastric cancer. Methods 96 cases of advanced gastric cancer were the research objects,46 cases treated by home enteral nutrition were the EN group,and 50 cases treated with parenteral nutrition were the PN group,the changes of nutritional status before and after nutritional support and quality of life after 6 months of the 2 groups were compared. Results The blood composition and the body composition after nutritional support of the 2 groups compared with before nutritional support had obvious difference( P <0. 05),and there was no significant difference in blood components between the 2 groups( P > 0. 05),but the body weight,BMI and fat mass of patients in EN group were significantly higher than those of the PN group( P < 0. 05),and the KPS,OLI and functional scores of EORTC QLQ C30 EN of EN group were significantly higher than those of the PN group,while the single score of EORTC QLQ C30 was significantly lower than that of the PN group( P < 0. 05),the differences were statistically significant. Conclusion Home enteral nutrition can effectively improve the nutritional status of advanced gastric cancer,and compared with parenteral nutrition,it has better effect in improving quality of life of patients,it is worthy of application in advanced gastric cancer patients.
