术后气管感染对原发性喉癌患者肺部功能及复发的影响

The Impact of Postoperative Tracheal Infection on Pulmonary Function and Recurrence of Primary Laryngeal Carcinoma Patients

目的研究术后气管感染对原发性喉癌患者肺部功能及复发的影响。方法选择实施喉癌手术的患者200例,观察并记录原发性喉癌患者术后的气管感染情况,并对术后气管感染患者肺部功能及复发的结果进行分析。结果实施喉癌切除术的患者的气管感染率为9.50%(19例),其中革兰阴性菌感染12例,感染率63.16%;革兰阳性菌感染7例,感染率为36.84%。术后发生气管感染患者的FEV1%、FEV1、MMEF、V50值均高于手术前;V25、FVC值均低于手术前,其中FEV1%、V25术前术后差异具有统计学意义,其余指标差异均无统计学意义。200例喉癌切除患者,发生气管感染患者的复发率为15.79%,未感染组的复发率为11.60%,2组结果相比,差异无统计学意义。结论术后气管感染对原发性喉癌患者肺部功能有一定的影响,且肿瘤复发率较高,在临床上应尽可能采取措施降低感染的发生率,提高原发性喉癌患者手术的治疗效果。

Objective To study the impact of postoperative tracheal infection on pulmonary function and recurrence of primary laryngeal carcinoma patients. Methods 200 cases of patients treated with laryngeal surgery were selected. Tracheal infection of primary laryngeal carcinoma patients observed and recorded,and to analyze the function and the results of tracheal recurrence of lung infection. Results Patients treated with laryngeal surgery infection rate was 9. 5%( 19 cases),Gram-negative bacterial infections was 12 cases,the infection rate was 63. 16%; 7 cases of gram-positive bacterial infections,the infection rate was 36. 84%,Postoperative infections in patients with tracheal FEV1%,FEV1,MMEF,V50 values were higher than before surgery;V25,FVC values were lower than before surgery,FEV1%,V25 difference was statistically significant,other indicators showed no statistical significance. 200 cases of laryngeal cancer resection patients tracheal infection recurrence rate was 15. 79%,uninfected group recurrence rate was 11. 60%,compared to the results of the 2 groups,the difference was not statistically significant. Conclusion Tracheal infection has certain impact on primary pulmonary function in patients with laryngeal cancer,and some patients have a higher rate of recurrence of symptoms,We should take measures to reduce clinical incidence of infection,improve the treatment effects of patients with primary laryngeal surgery.
