调强放疗联合TP化疗对晚期食管癌患者血清microRNA-21水平及预后的影响

Effect of Intensity Modulated Radiation Therapy Combined with TP Chemotherapy on Serum microRNA-21 Level and Prognosis in Patients with Advanced Esophageal Cancer

目的探讨调强放疗联合紫杉醇-顺铂(TP)化疗对晚期食管癌患者血清微小RNA-21(microRNA-21)水平及预后的影响。方法选取晚期食管癌患者300例,采用随机数表法随机分为观察组(n=150)与对照组(n=150)。治疗前,检测两组患者血清microRNA-21水平。对照组患者给予TP化疗,观察组在此基础上给予调强放疗。治疗后再次检测两组患者血清microRNA-21水平。结果治疗前两组患者血清microRNA-21水平比较无显著差异(P> 0. 05)。治疗后,观察组血清microRNA-21水平低于对照组,而KPS评分、治疗有效率、疾病控制率均高于对照组,差异均具有统计学意义(P <0. 05)。观察组中位生存期与总生存率均高于对照组,血清microRNA-21低水平组患者的中位生存期与总生存率均高于血清microRNA-21高水平组,差异具有统计学意义(P <0. 05)。结论调强放疗联合TP化疗可显著降低晚期食管癌患者的血清microRNA-21水平,并提高患者生存率。

Objective To investigate the effects of intensity modulated radiation therapy combined with paclitaxel-cisplatin(TP)chemotherapy on serum microRNA-21 levels and prognosis in patients with advanced esophageal cancer.Methods 300 patients with advanced esophageal cancer were randomly divided into observation group(n=150)and control group(n=150).Before treatment,the serum basal microRNA-21 levels of the two groups were detected.Control group patients were given TP chemotherapy,and the observation group was given intensity modulated radiation therapy on this basis.After treatment,serum microRNA-21 levels were detected again.Results Before treatment,there was no significant difference in serum basal microRNA-21 level between the two groups(P>0.05).After treatment,the serum microRNA-21 level of the observation group was lower than that of the control group,while the KPS score,treatment efficiency and disease control rate were higher than those of the control group,the difference was statistically significant(P<0.05).The median survival time(MST)and overall survival(OS)in the observation group were higher than those in the control group.MST and OS in the low serum microRNA-21 group were higher than those in the high serum microRNA-21 group(P<0.05).Conclusion Intensity modulated radiation therapy combined with TP chemotherapy can significantly reduce the serum microRNA-21 level in patients with advanced esophageal cancer,and improve the survival rate of patients.
