上海桃浦镇社区糖尿病门诊患者失访原因分析

Analysis of the reasons for missing follow-up diabetes patients in the diabetes clinic of Taopu community health service centre of Shanghai

目的:分析桃浦镇社区糖尿病门诊患者的失访原因。方法:跟踪随访我中心糖尿病门诊纳入信息管理系统的719名糖尿病患者,对其中106例失访者通过电话调查其失访原因。结果:失访率为16.0%,失访原因中前4位分别为:到上级医院就诊(34.5%),药店自己买药(31.9%),经济困难(10.3%),服用保健品(10.3%)。结论:糖尿病电子管理系统可帮助社区医生提高管理效果,但更重要的是加强糖尿病患者健康教育,提高患者的就医依从性,减少失访率。

Objective: To investigate the reasons for missing follow-up diabetes patients in Taopu community diabetes clinic. Methods: 719 diabetic patients were followed up, who were booked into our diabetes information management system. But among them, 106 patients who were missed follow-up were enquired the reasons by phone-call. Results: The rate of missing follow-up was 16%. The main four reasons were referring to higher grade hospitals(34.5%), getting the medicine at the pharmacy by themselves(31.9%), economic hardship( 10.3%) and taking health care products(10.3%), respectively. Conclusion:(1)Diabetes information management system can help community doctors to detect the subjects who have missed follow-up in time.( 2)Improving medical services, strengthening diabetes education and good communication between doctors and patients can enhance patient compliance and reduce the rate of missing follow-up.
