雌激素乳膏辅助治疗老年阴道炎的临床疗效及对血清学指标的影响

Clinical effect of adjuvant estrogen cream on elderly patients with vaginitis

目的探讨雌激素乳膏辅助治疗老年阴道炎的临床疗效及其对血清学指标的影响。方法回顾性选取2015年2月至2017年9月在我院治疗的老年阴道炎病人100例,所有病人依据治疗方法分为对照组(48例)和观察组(52例),对照组给予甲硝唑阴道凝胶治疗,观察组在对照组的基础上给予雌激素乳膏,对2组病人临床疗效及血清学指标进行评估。结果对照组治疗后总有效率为85. 41%,观察组治疗后总有效率为98. 08%,组间比较差异有统计学意义(P<0. 05);治疗后,观察组阴道分泌物减少时间、瘙痒感缓解时间、灼痛感缓解时间及阴道充血消退时间分别为(6. 40±0. 92) d、(4. 70±0. 46) d、(3. 54±0. 41) d及(7. 74±0. 72) d,均显著低于对照组(P<0. 05);治疗后,2组病人雌二醇(E2)水平及子宫内膜厚度均较治疗前明显升高,促卵泡激素(FSH)及促黄体激素(LH)水平较治疗前明显降低,观察组病人E2水平及子宫内膜厚度分别为(23. 34±2. 13) pg/L及(2. 71±0. 72) mm,均明显高于对照组,FSH及LH水平分别为(60. 49±0. 84) IU/L及(20. 78±7. 89) IU/L,均明显低于对照组,组间及组内比较差异均有统计学意义(P<0. 05)。结论雌激素乳膏辅助治疗能有效改善老年阴道炎病人临床症状及血清激素水平,降低病人不适,进而促进病人康复。

Objective To investigate the clinical efficacy and effect on serological indicators of estrogen cream adjuvant in patients with senile vaginitis. Methods A total of 100 elderly patients with vaginitis treated in our hospital from February 2015 to September 2017 were retrospectively selected. All patients were divided into control group( n = 48) and observation group( n = 52) according to the treatment methods. The patients in control group were given Metronidazole gel. The observation group was given estrogen cream on the basis of the control group,and then the clinical curative effect and serological indexes of the two groups were evaluated and compared. Results The total effective rate of the control group was 85. 41%,which was significantly higher than that of the control group( 98. 08%)( P<0. 05).After treatment,the time of reducing vaginal discharge,pruritus,burning pain relief and remission of vaginal congestion in the observation group were( 6. 40 ± 0. 92) d,( 4. 70 ± 0. 46) d,( 3. 54 ± 0. 41) d and( 7. 74 ± 0. 72) d respectively,which were significantly lower than those in the control group( P<0. 05). After treatment,the levels of E2 and endometrial thickness in both groups were significantly higher than those before treatment,while the levels of FSH and LH were significantly lower than those before treatment. The levels of E2 and endometrial thickness in the observation group were significantly higher than those of the control group. The levels of FSH and LH were significantly lower than those of the control group( P< 0. 05). Conclusions Estrogen cream adjuvant therapy can effectively improve the clinical symptoms and serum hormone levels in elderly patients with vaginitis,reduce the discomfort of patients,and then promote the rehabilitation of patients.
