Th17细胞相关细胞因子IL-17/IL-23与湿疹发病及病情严重程度的关系 被引量:32

The associations of Th17 cell-relevant cytokines IL-17/IL-23 with the pathogenesis and disease severity of eczema

摘要 目的新型Th17细胞及其相关细胞因子IL-17/IL-23在炎症性过敏性皮肤病中具有重要作用,但与湿疹的关系尚不明确,本研究目的是探讨Th17细胞相关细胞因子IL-17/IL-23与湿疹发病及病情严重程度的关系。方法选取60例湿疹患者及28例健康对照者,所有患者进行EASI评分,根据评分将湿疹患者分为轻、中、重度3组(各20例)。采用ELISA检测血清IL-17和IL-23水平,分析其与EASI评分、嗜酸性粒细胞计数(EOS)及血清总IgE的关系。结果湿疹患者血清IL-17、IL-23水平显著高于正常对照组,IL-17、IL-23水平呈正相关。轻、中、重度湿疹患者与正常对照组血清IL-17、IL-23水平差异有统计学意义,重度湿疹患者血清IL-17、IL-23水平显著高于轻、中度,中度患者高于轻度,轻度患者高于正常对照,差异均有统计学意义(P<0.05);血清IL-17、IL-23水平与EASI评分呈正相关,提示血清IL-17、IL-23水平与湿疹严重程度相关。EOS计数升高湿疹患者血清IL-17、IL-23水平显著高于EOS计数正常组,IL-17、IL-23水平与EOS计数呈正相关。IgE升高组湿疹患者血清IL-17和IL-23水平略高于IgE正常组,但差异无统计学意义。结论 IL-17/IL-23共同参与了湿疹的发病,且与疾病严重程度密切相关。IL-17/IL-23可能与嗜酸性粒细胞增多有关,但与血清总IgE关系有待进一步研究。 Objective It is revealed that Th17 cells and their relevant cytokines interleukin-17( IL-17) and interleukin-23( IL-23) play crucial roles in the inflammatory allergic skin diseases. But their roles in eczema are still unidentified. We aim to investigate the possible roles of Th17 cell-relevant cytokines IL-17/IL-23 for the pathogenesis and severity assessment of eczema in this study. Methods Sixty cases of patients with eczema and 28 sex-and age-matched healthy controls were recruited. According to eczema area and severity index( EASI) score,patients with eczema were divided into mild,moderate and severe ones( 20 cases respectively). Concentrations of IL-17 and IL-23 in serum were assessed by enzymelinked immunosorbent assay( ELISA),and their associations with EASI score,eosinophil( EOS) count and total IgE level were analyzed. Results Compared with healthy controls,serum concentrations of IL-17 and IL-23 in patients with eczema were significantly elevated. And the level of IL-17 correlated positively to that of IL-23. The serum levels of IL-17 and IL-23 in patients with mild,moderate,severe eczema and healthy controls were significantly different,severe eczema higher than mild and moderate ones,moderate eczema higher than mild ones; and mild ones higher than controls( P < 0. 05).Meanwhile,serum levels of IL-17 and IL-23 in patients with eczema positively correlated to EASI scores,indicating that IL-17 and IL-23 levels were associated with disease severity of eczema. The levels of IL-17 and IL-23 in eczema patients with elevated EOS count were significantly higher than that of normal EOS count. Beside,IL-17 and IL-23 levels of patients with eczema showed positive correlations to EOS count. Enhanced IL-17 and IL-23 levels were observed in eczema patients with elevated total IgE than that of normal IgE,but with no significant differences. Conclusion IL-17 and IL-23 may play synergetic roles in the pathogenesis and severity assessment of eczema. IL-17/IL-23 are probably correlated to the elevated EOS count in eczema,but the relationships with total IgE need further studies.
