摘要
目的观察2型糖尿病患者脑梗死急性期血小板表达PECAM-1、P-选择素的改变,并观察抗血小板制剂对其表达的影响。方法采用全血法流式细胞术检测38例2型糖尿病急性脑梗死(ACI)患者血小板表达PECAM-1、P-选择素的水平,与非糖尿病ACI患者(40例)比较;将38例患者分为常规治疗组20例和奥扎格雷钠治疗组18例,于发病48h、7d、14d各采集静脉血进行检测;治疗前、治疗后7d、14d分别进行爱丁堡+斯堪的那维亚神经功能缺损程度评分及残疾水平Barthel指数评定。结果3组研究对象PECAM-1、P-选择素间差异有统计学意义(P<0.01);P-选择素3组间两两比较差异均有统计学意义(P<0.01);奥扎格雷钠治疗组与常规治疗组治疗7d、14d后PECAM-1、P-选择素间差异均有统计学意义(P<0.05)。奥扎格雷钠治疗组与常规治疗组患者治疗7d和14d后神经功能缺损程度评分、Barthel指数间差异均有统计学意义(P<0.01)。结论2型糖尿病ACI患者血小板P-选择素表达明显高于非糖尿病ACI患者,PECAM-1表达在糖尿病组与非糖尿病组无明显差异;奥扎格雷钠治疗能加快ACI患者P-选择素的下降,减慢PECAM-1的下降。奥扎格雷钠能减轻ACI患者神经功能缺损症状,降低残疾水平。
Objective To detect the levels of PECAM-1 and P-selectin expressed by platelets in patients with 2-type diabetes with acute cerebral infarction(ACI),to observe the effect of Sorium Ozagrel on ACI and on expression of PECAM-1,P-selectin by platelets in ACI patients with diabetes.Methods Platelet PECAM-1 and P-selectin was detected by whole blood flow cytometry.The 38 patients were divided into 2 groups randomly:① common therapy group(n=20) ②Sodium Ozagrel therapy group(n=18).ACI Patients were given scores of nervous functional deficiency and Barthel index before treated and on days 7,14 after treated.Results For ACI patients,the expression levels of P-selectin in patients with diabetes were higher than those of patients without diabetes,P<0.01;expression levels of PECAM-1 had no difference between ACI patients with diabetes and ACI patients without diabetes.On days 7,14 after Ozagrel treatment,the scores of nervous functional deficiency and Barthel index improved significantly compared to common therapy groups;the levels of PECAM-1declined more slowly,while the level of P-seletin declined more faster,than those of common therapy.Conclusion Expression levels of platelets P-selectin increased significantly in ACI patients with diabetes,expression levels of PECAM-1 had no difference between ACI patients without diabetes and ACI patients without diabetes.Ozagrel treatment could accelerate the decline of P-selectin expression and decelerate the decline of PECAM-1 expression;Ozagrel treatment could alleviate the degree of nervous functional deficiency and the disability of ACI patients.
