摘要 目的观察急性脑梗死并出血性转化(HT)患者血清补体C3水平变化,并分析其临床意义。方法选取2010年3月—2014年5月在广西医科大学第十附属医院神经内科住院治疗的急性脑梗死患者68例,根据HT发生情况分为HT组和非HT组,每组34例,检测两组患者入院第1、2、3、7、14、21天血清补体C3水平。结果两组患者血清补体C3水平均是先升高并在入院第7天后开始下降,到入院第21天时恢复到入院水平,而HT组患者血清补体C3水平高于同期非HT组,以入院第1、2、3天最为明显,表现为急剧升高。结论急性脑梗死并HT患者初期血清补体C3水平持续升高,补体系统的激活/抑制可能是导致急性脑梗死患者出现HT的重要原因。 Objective To analyze the change and clinical significance of serum complement C3 level of acute cerebral infarction patients complicated with hemorrhagic transformation. Methods From March 2010 to May 2014,a total of 68 patients with acute cerebral infarction were selected in the Department of Neurology, the 10 th Affiliated Hospital of Guangxi Medical University,and they were divided into A group( complicated with hemorrhagic transformation) and B group( did not complicated with hemorrhagic transformation) according to the incidence of hemorrhagic transformation,each of 34 cases. Serum complement C3 level was detected after 1 day,2 days,3 days,7 days,14 days and 21 days of admission. Results Serum complement C3 level of the two groups elevated after admission and gradually reduced after 7 days of admission,then recover to similar level with that at admission,the serum complement C3 level of A group was statistically significantly higher than that of B group at different time points, respectively, especially after 1 day, 2 days, 3 days of admission, it performed as acute elevation. Conclusion Serum complement C3 level of acute cerebral infarction patients complicated with hemorrhagic transformation obviously and continuously elevated,activation / inhibition of complement system may play an important role in the hemorrhagic transformation in patients with acute cerebral infarction.
