胆管出血的原因诊断与治疗效果研究

目的分析胆管出血的原因诊断与治疗效果。方法选取2005年6月—2017年2月北海市中医医院收治的胆管出血患者50例作为研究对象,根据不同治疗方法分为观察组(n=30)和对照组(n=20)。分析50例胆管出血患者出血原因。比较两组患者止血效果、临床疗效、随访结果。结果 31例(62.0%)患者为胆管结石合并感染,其次依次位创伤性胆管出血11例(22.0%)、胆肠吻合术4例(8.0%)、胆管肿瘤(6.0%)、肝脓肿合并胆管感染1例(2.0%)。观察组患者止血有效率(93.3%)高于对照组(85.0%)(P<0.05)。两组患者临床疗效、再次胆管出血发生率比较,差异无统计学意义(P>0.05)。结论针对胆管出血患者需要在了解其出血原因的基础上加强诊断,考虑患者个体情况选择合适治疗方式,只有这样才可能获得满意效果。

Objective To analyze the cause,diagnosis and treatment of biliary tract bleeding. Methods From June2005 to February 2017,50 patients with bile duct bleeding in Beihai Hospital of Traditional Chinese Medicine were selected as the research objects. According to different treatment methods,the patients were divided into observation group( n = 30) and control group( n = 20). The causes of bleeding in 50 patients with biliary tract bleeding were analyzed. The hemostatic effect,clinical efficacy and follow-up results were compared between the two groups. Results 50 cases in 31 cases( 62. 0%) patients with bile duct infection, followed by 11 cases of traumatic hemobilia( 22. 0%),4 cases of biliary intestinal anastomosis( 8. 0%),biliary tumor( 6. 0%),liver abscess associated with biliary tract infection in 1 cases( 2. 0%). The effective rate of hemostasis in the observation group( 93. 3%) was higher than that in the control group( 85. 0%)( P < 0. 05). There was no significant difference between the two groups in the clinical efficacy and the incidence of re biliary bleeding( P > 0. 05).Conclusion For the patients with biliary tract bleeding,it is necessary to strengthen the diagnosis on the basis of understanding the cause of bleeding,consider the individual condition of the patient and choose the proper treatment method. Only in this way can the satisfactory result be achieved.
