超声消融治疗肌壁间、黏膜下和浆膜下子宫肌瘤的临床疗效比较研究

Comparative Study of Clinical Effect of Ultrasound Ablation in the Treatment of Intramural, Submucosal and Submucous Myoma of Fibroid

目的比较超声消融治疗肌壁间、黏膜下和浆膜下子宫肌瘤的临床疗效。方法选取自贡市第四人民医院2014年8月—2017年1月收治的肌壁间子宫肌瘤50例(肌壁间组)、黏膜下子宫肌瘤50例(黏膜下组)和浆膜下子宫肌瘤50例(浆膜下组),3组患者均采用超声消融治疗。比较3组患者治疗前后肌瘤体积、症状严重程度评分量表(SSS)评分及临床疗效、并发症发生率。结果治疗前3组患者肌瘤体积和SSS评分比较,差异无统计学意义(P>0.05);治疗后黏膜下组患者肌瘤体积小于肌壁间组和浆膜下组(P<0.05);治疗后3组患者SSS评分比较,差异无统计学意义(P>0.05)。3组患者临床疗效和并发症发生率比较,差异无统计学意义(P>0.05)。结论超声消融治疗肌壁间、黏膜下和浆膜下子宫肌瘤的临床疗效相当,安全性相似,均可改善患者临床症状,但黏膜下肌瘤体积消融率更高。

Objective To compare the clinical effect of ultrasound ablation in the treatment of intramural,submucosal and submucous myoma of fibroid. Methods A total of 50 cases of intramural fibroid as A group,50 cases of submucous fibroid as B group,50 cases of submucous myoma of fibroid as C group,Zigong Fourth People' s Hospital from August 2014 January2017,patients of three groups were treated by ultrasonic ablation. The volume of myoma and SSS score before and after treatment,clinical efficacy and complication rate were compared among the three groups. Results No statistically significant difference of myoma volume and SSS score among the three groups before treatment( P > 0. 05). After treatment,myoma volume of B group were statistically smaller than those of A and C groups( P < 0. 05); no statistically significant difference of SSS score among the three groups after treatment( P > 0. 05). No statistically significant difference of clinical efficacy and complication rate among the three groups( P > 0. 05). Conclusion The clinical effect of ultrasound ablation for intramural,submucosal and submucosal fibroids is comparable,it can improve the clinical symptoms of patients with high safety, but the rate of submucosal myoma volume ablation is higher.
