南京市高淳地区幽门螺杆菌感染现状及危险因素分析

Helicobacter pylori infection status and risk factors analysis in Gaochun area of Nanjing

目的研究江苏省南京市高淳地区幽门螺杆菌(Hp)的感染分布情况及其发生危险因素的探讨分析。方法回顾性分析2015年1月至2018年2月我院Hp检查室进行Hp检测及治疗的1 367名体检及门诊患者检查资料。从检查者的性别、年龄、吸烟、饮酒、检查通道、检查底气、样气^(13)C浓度、饮食前手卫生、外出就餐、经济情况、文化程度、收入、口味及是否医务人员等相关指标进行分组分析。结果高淳地区2015—2018年初的幽门螺杆菌感染率为35.78%,Hp感染与性别、不同年龄段、不同检测通道、检查底气^(13)C浓度、不同文化程度以及不同家庭收入水平无明显关联,差异无统计学意义;Hp感染阳性与吸烟、饮酒、饭前不洗手、经常外出就餐、饮食口味偏重情况、为医务人员、检查样气^(13)C浓度明显高有明显关联性,差异有统计学意义;回归分析提示吸烟、饮酒、饮食前手卫生情况、是否经常外出就餐、饮食口味、是否为医务人员为导致人群Hp阳性的危险因素;吸烟对于Hp阳性的影响最大。结论南京市高淳地区Hp感染率低于全国平均水平,建立地方相关Hp感染数据档案能提供当地人群Hp感染流行数据,为防治Hp感染提供指导作用。

Objective To investigate the region distribution of Helicobaeter pylori infection and evaluate related risk factors Gaochun area of Nanjing.Methods 1 367 people who had undergone health check-ups or outpatientat at Nanjing Gaochun people′s hospital Hospital from January 2015 to february 2018 were examined for Helicobacter pylori infection by the 13C breath test.Data including gender,age,smoking,drinking,detection channel,the density of bottom and sample gas,washing before eating,eating out,the degree of education,the economy income,tastes and whether for hospital staff.Risk factors for Hp infection were analyzed.Results The overall Hp infection rate was 35.78%.Hp infection rates showed differences among smoking,drinking,the density of sample gas,washing before eating,eating out,tastes and whether for hospital staff.No significant relationship was found between gender,age,detection channel,the density of bottom gas,the degree of education,the economy income.Logistic regression analysis suggest smoking,drinking,washing before eating,eating out,Heavy tastes,whether for hospital staff are in Hp positive risk factors;The influence of smoking on Hp positive is the largest.Conclusion Gaochun area of Nanjing Hp infection rates below the national average,establish a local Hp infection related data archive can provide Hp infection prevalence data of the local population and provide the strengthen guidance for prevention or treatment of Hp infection.
