整治手法治疗颈型颈椎病对颈椎曲度影响临床观察

Clinical Observation on Effect of Remediation Technique Treatment Neck Type of Cervical Spondylosis of the Cervical Physiological Curvature

[目的]观察整治手法治疗颈型颈椎病疗效。[方法]对42例颈曲改变颈椎病门诊患者采用整治手法(舒筋理筋:患者取坐位,采用捋按、拿按理筋,拿捏颈肌,揉滚按摩手法,使颈肩部肌肉放松,同时测摸痛点和病变部位;端提牵引:患者取坐位,医者站其旁侧,左手掌放于患者下颌部,另一手虎口放于枕后下方,两手相对用力,保持头部屈曲约20°,并稳准向上用力端提约1min,可反复2～3次;坐位前推:根据X片提示,结合手部触诊情况,医者站于患者后方,左肘关节外侧勾托患者头部,胸前紧贴患者头后部,右手拇指按压在患椎棘突上,左肘关节将头平稳用力上提牵引,在此过程中将头后仰上提,右手拇指将患椎节段向前或偏移的侧方推顶,常可听到"咯"的一声,手法完成。连续治疗14d为1疗程。观测临床症状、颈椎曲度、不良反应。连续治疗2疗程,判定疗效。[结果]总有效率36例(85.7%)。Ⅱ型改善程度最高,颈曲改善率94%,Ⅳ型改善程度最低,改善率50%。[结论]整治手法在改善颈椎内外平衡中起重要作用,颈曲重建在颈椎病治疗中具有重要的价值。

[Objective]To observe remediation practices,the treatment of the neck cervical spondylosis.[Methods]42 cases of cervical spondylosis cervical curvature change outpatients using remediation techniques(Shujin tendon: the patient sitting,using stroking press,take reasonably tendons,just the right neck muscle,kneading roller massage,neck and shoulder muscles to relax so that while measuring touch pain points and lesions;side mention traction: The patient was sitting,medical station its flanking,put his left hand on the patient jaw with one hand placed on the pillow beneath the tiger's mouth,hands relatively hard,keep your head flexion of about 20°,and steady quasi-forced end up mentioning about 1min,can be repeated 2 to 3 times;seat forward and backward: According to X-rays tips,combined with hand palpation situation,medical patients who stand on the rear,left lateral elbow hook care patient's head,chest tight stickers patients with head and back,right thumb pressed against the risk of vertebral spinous process,the left elbow to put on the head steady traction force,in this process will be put on the head thrown back,his right thumb will move forward or risk of vertebral segment offset the side of the ejector,you can often hear ' slightly' sound,the way complete.continuous treatment 2 weeks(14d) as a course of treatment.observation of clinical symptoms,cervical curvature,adverse reactions.continuous treatment of two courses,to determine efficacy.[Results]The total effective rate was 36 cases(85.7%).Type II the highest degree of improvement,of cervical curvature improvement rate of 94%,the lowest type IV improvement,the improvement rate of 50%.[Conclusion]remediation techniques play an important role in improving the cervical internal and external balance,the cervical curvature rebuild an important value in the treatment of cervical disease.
