蓬草除痹汤对类风湿性关节炎大鼠TNF-α、IL-1β、前列腺素E2水平、RANKL及护骨素影响随机平行对照研究

Clinical Study of Tongbi Decoction in the Treatment of Rheumatoid Arthritis in Rat Bone Tissues Except Peng Grass

[目的]观察蓬草除痹汤对类风湿性关节炎大鼠细胞因子及骨吸收和重建影响。[方法]使用随机平行对照方法,将60只Wistar大鼠编号查随机数字表分为六组,空白组、模型组、蓬草除痹汤(高、中、低剂量)组、雷公藤多甙片组,10只/组。复制胶原诱导性关节炎大鼠模型,分别灌胃干预,空白组、模型组等容积生理盐水;蓬草除痹汤组(生药38.5g/kg,19.5g/kg,9.75g/kg),雷公藤多甙片混悬液5.75g/kg,1次/d,连续28d。连续干预4周后,各组大鼠眶静脉取血,3500r/min,离心10min,取上清液存于-70℃冰箱备用。观测关节炎指数、白细胞介素1β、肿瘤坏死因子α及前列腺素E2水平、光镜下RANKL、护骨素。[结果]各药物干预组均可显著抑制大鼠右足肿胀程度(P<0.01/P<0.05),血清白细胞介素1β、肿瘤坏死因子α及前列腺素E2水平均明显低于模型组(P<0.01);蓬草除痹汤中、高剂量组抑制作用明显优于雷公藤多甙片组(P<0.05);各药物干预组大鼠关节软骨下骨和骨小梁区核因子κB受体活化因子配体表达均低于模型组(P<0.01),护骨素关节软骨组织表达量均低于模型组(P<0.01)。[结论]蓬草除痹汤可有效改善胶原诱导性关节炎大鼠关节炎症状,减轻骨侵蚀,机制与下调血清中致炎因子白细胞介素1β、肿瘤坏死因子α及前列腺素E2的水平,抑制类风湿性关节炎大鼠关节中核因子κB受体活化因子配体表达有关。

[Objective]To observe Peng Cao rheumatism-syndrome dispelling decoction’s effect on cytokines,bone resorptionand remodeling of rats with rheumatoid arthritis.[Methods]Sixty Wistar rats were randomized into six groups: blank group,model group,high-,middle-and low-dosagePeng Cao rheumatism-syndrome dispelling decoction groups and Tripterygiumwilfordiigroup.Except blank and modelgroup,other groups were administrated intragastrically once a day for 28 days.[Results]Each group had a remarkable effect on relieving rightfoot swelling(P<0.01or0.05).The levels of serum interleukin 1β,prostaglandin E2,tumor necrosis factor α weresignificantly lower in the treatment groups than the model group(P<0.01).The middle-and highdose Peng Caorheumatism-syndrome dispelling decoction had a better effect than Tripterygiumwilfordii(P<0.05).The expressions ofreceptor activator of nuclear factor kappa B and osteoprotegerin were significantly lower in the treatment groups than themodel group(P<0.01).[Conclusion]These findings indicate that Peng Cao rheumatism-syndrome dispelling decoction is capable ofimproving symptoms of rheumatoid arthritis effectively and restraining bone erosion.The mechanism may relate to the following aspects:①Reduction in levels of interleukin 1β,prostaglandin E2,tumor necrosis factor α in serum.②Expression restraint on receptor activator of nuclear factor kappa B ligand in the joint of rats with rheumatoid arthritis.
