艾灸、热敷护理腹腔镜胆囊切除术围手术期

Moxibustion,Heat treatments Nursing on Perioperative Patients with Laparoscopic Cholecystectomy

[目的]观察艾灸、热敷护理腹腔镜胆囊切除术围手术期疗效。[方法]使用随机平行对照方法,将61例住院患者按就诊顺序编号法随机分为两组。对照组31例常规术后护理;治疗组30例术后6 h艾灸、中药热敷护理。①艾条回旋灸,取足三里穴,点燃艾条一端,对准应灸腧穴部位,约距皮肤2～3 cm,熏烤,患者局部有温热感无灼痛为宜,灸10～15 min,至皮肤出现红晕,连灸3 d。②中药热敷:吴茱萸及粗盐各250g放入布袋中混合加热至烫手放于患者腹部热敷20 min,第2、第3天每日上午、下午各热敷1次。常规护理同对照组。连续护理3d为1疗程。观测临床症状、肠鸣音恢复时间与肛门排气时间、出现肠胀气及使用甘油灌肠剂、不良反应。护理1疗程,判定疗效。[结果]治疗组痊愈11例,显效18例,无效1例,总有效率96.70%;对照组痊愈5例,显效19例,无效6例,总有效率80.00%。治疗组疗效优于对照组(P<0.05)。术后肠鸣音恢复时间和肛门排气时间治疗组较对照组明显提前(P<0.05);术后胃肠道不适症状治疗组较对照组明显减少或减轻(P<0.05)。[结论]艾灸、热敷护理促进腹腔镜胆囊切除术胃肠功能恢复并减轻术后胃肠道不适,效果显著。

[Objective]Observe Moxibustion,Heat treatments nursing on perioperative patients with laparoscopic cholecystectomy.[Methods] Using randomized controlled method,61 cases of hospitalized patients according to treatment were randomly divided into two groups sequentially numbered.Control group,31 cases of routine postoperative care;treatment group,30 cases 6 h after moxibustion,herbal heat treatments.① moxa moxibustion roundabout,take the Zusanli,ignited moxa end,the alignment should moxibustion acupoint location,away from the skin about 2 ~ 3 cm,for smoked,so that patients feel warm without burning locally appropriate,each moxibustion at 10 ~ 15 min,to flush the skin for the degree,the first two days,the first three days even moxibustion 3 day.② Herbal Compress: Evodia and coarse salt into the bag of 250g mixed and heated to hot put in the patient's abdomen hot 20 min,2,3 days daily morning and afternoon heat 1.Routine care with the control group.3day is a continuum of care treatment.Observation of clinical symptoms,bowel sounds and flatus recovery time,flatulence and the use of glycerin enema agent situation,adverse reactions.Nursing a course of treatment,to determine efficacy.[ Result] The cure 11 cases,effective in 18 cases,ineffective in one case,the total efficiency of 96.70%;recovered five cases of the control group,19 cases,6 cases,the total effective rate 80.00%.Treatment group than the control group(P <0.05).Patients treated with postoperative recovery time of bowel sounds and flatus significantly earlier than the control group(P <0.05);postoperative gastrointestinal symptoms were markedly reduced or mitigated.[ Conclusion] Moxibustion,Heat treatments nursing can promote gastrointestinal function recovery and reduce postoperative gastrointestinal discomfort on patients with laparoscopic cholecystectomy.
