中医护理配合推拿按摩、牵引、针刺、微波、电疗及中药熏蒸治疗腰椎间盘突出症随机平行对照研究

TCM Care with Massage,Traction,Acupuncture,Microwave,Electrotherapy and Fumigation Treatment of Lumbar Disc Herniation Randomized Controlled Study

[目的]观察中医护理配合推拿按摩、牵引、针刺、微波、电疗及中药熏蒸治疗腰椎间盘突出症疗效。[方法]使用随机平行对照方法,将100例门诊患者按随机数字表法随机分为两组。均推拿按摩、针刺、微波理疗、调制中频电疗及中药熏蒸。常规护理组47例常规护理。中医护理组53例中医护理:①观察护理。观察患者的病情及个体特征,制定有针对性的、完整的护理计划。急性期应严格卧硬板床休息3～4周,半年内不做中等以上体力劳动;避免俯卧,以防诱发疼痛,翻身时注意保持躯干一致,切记勿脊柱扭转或屈曲。②健康教育。帮助患者认识和了解该病发病机理和注意事项,认真解答患者疑惑。腰椎间盘突出患者因腰痛以及下肢疼痛麻木,常常会紧张、恐惧,担心自己将生活无法自理,产生焦虑自卑情绪,因此,要做好对其情志护理,增加患者信心,使其主动配合治疗。③功能锻炼。指导患者进行适当运动,帮助其恢复身体机能。常规护理同常规护理组。连续治疗6d为1疗程。观测临床症状、不良反应。连续治疗3疗程,判定疗效。随访1年25例。[结果]中医护理组治愈26例,有效24例,无效3例,总有效率94.30%。常规护理组治愈14例,有效25例,无效5例,总有效率83.00%。中医护理组疗效优于常规护理组(P<0.05)。[结论]中医护理配合推拿按摩、牵引、针刺、微波、电疗及中药熏蒸治疗腰椎间盘突出症效果显著,值得推广。

[ Objective] Observation and nursing care medicine conservative treatment of lumbar disc herniation.[Methods] Using randomized controlled method,100 outpatients were randomly divided into two groups randomly.Are massage,acupuncture,microwave therapy,modulation frequency electrotherapy and fumigation.Usual care group,47 cases of routine care.Group of 53 cases of Chinese medicine nursing care:① observation of care.Observe the patient's condition and individual characteristics,to develop targeted and complete care plan.Acute phase should be strictly lying Banchuang rest 3 to 4 weeks,six months,not more than moderate physical labor;avoid the prone position,to prevent induced pain,when turning to keep the trunk line,remember not to twist or bend the spine.② Health Education.Help patients to recognize and understand the pathogenesis of the disease and precautions,serious answers were doubts.Patients with low back pain and lumbar disc pain and numbness in the lower limbs,often tense,fear,worried that they will not be able to live self-care,anxiety feelings of inferiority,therefore,to make its emotional care,increase patient confidence to actively cooperate with treatment.③ functional exercise.Proper exercise guide patients to help them restore bodily functions.Routine care with routine care group.6d is a continuous course of treatment.Observation of clinical symptoms and adverse reactions.Continuous treatment of 3 courses,to determine efficacy.Followed for 1 year 25 cases.[ Results] TCM treatments were cured 26 cases,effective in 24 cases,3 cases,the total efficiency of 94.30%.Usual care group were cured 14 cases,effective in 25 cases,5 cases,the total efficiency of 83.00%.Chinese nursing group than the usual care group(P <0.05).[ Conclusion] Medicine care with massage,traction,acupuncture,microwave,electrotherapy and medicine fumigation treatment of lumbar disc herniation significant effect,worthy of promotion.
