肾病综合征常以蛋白尿待诊,须详辨证侯。脾阳不运,面黄浮肿、神倦、纳少、便溏,实脾饮;肾阳虚衰,面晄白,身倦怯寒,四肢欠温、夜尿频、浮肿甚、舌胖嫩,真武汤;肺卫气虚,面目浮肿,时因外感加重,玉屏风散合防己茯苓汤;精血匮竭,面色无华、发枯齿折、遍身浮肿、舌质嫩,质淡有齿痕,安肾丸;阴虚阳亢,久服西药激素,见面红、咽赤、舌质红、血压增高、脉弦数,养阴渗湿汤;湿热伤阴,久服激素药、面红、多汗、舌质红、苔黄厚而腻,养阴泻火汤;痰气互结,久服激素无效、面红而晦、舌质紫暗,化坚二陈汤;肾脏虚损,双肾萎缩、血压升高、少苔、脉沉细迟,一贯煎加地龙、水蛭、藏红花;西药副作用甚者,可中药减之,顽症可消痰软坚、散结等逆转病理改变后扶助正气、温补以善其后,巩固疗效。谨避风邪、防止外感,应及时控制,否则常可危及生命。
