王一庆教授认为慢性萎缩性胃炎(CAG)的病机特点是虚实夹杂,虚中有实,实中有虚,既存在因实致虚,也有由虚致实的情况。临床治疗上,虚实要有侧重点,多数患者属于虚为主证,实为次证,虚证患者主要是气虚或是气阴两虚,实证不外乎是气滞、瘀血、痰浊、痰湿等。气虚患者血运不畅,脾脏气血不足运化无力,水谷精微无法布达全身,故全身气血更为匮乏。阴虚患者偏于胃,胃脏失养则导致胃络阻滞。虚证患者选用黄芪联合党参着重补气,同时加用黄精滋补阴液,茯苓、白术健脾运气。阴虚过于严重选用石斛、麦冬胃脘寒凝冷痛添用干姜、饴糖;嗳气多用旋复花、甘松、佛手等理气降逆;腹部胀满者用苍术、陈皮化湿。不可拘泥于一种症候,标本虚实需融会贯通,治虚不忘实、治实不忘虚,方可治疗得当。
