清热泄浊化瘀汤治疗湿热瘀阻型痛风34例临床观察

To Observe the Curative Effect of Heat Leakage Turbidity Decoction Treating Damp-heat Stasis Gout

[目的]观察清热泄浊化瘀汤治疗湿热瘀阻型痛风疗效。[方法]回顾性分析34例住院患者自拟清热泄浊化瘀汤(土茯苓、威灵仙各20g,白术、当归、党参、生黄芪、法半夏、陈皮、大黄、泽泻、忍冬藤、鸡血藤、蛇舌草、山慈菇、车前子、石韦、生薏仁、红花各10g。急性发作加芒硝、生地黄、知母、雷公藤各10g;痛风缓解期加仙茅10g,淮山药30g,补骨脂20g;痛风石加穿山甲、赤芍各10g,金钱草40g,蜈蚣3g)。1剂/d,水煎300mL,早晚口服。连续治疗14d为1疗程。观测临床症状、血尿酸浓度、不良反应。连续治疗2疗程(28d),判定疗效。[结果]34例患者中,显效13例,有效18例,无效3例,总有效率91.17%。血尿酸指标治疗前后数据差异显著性(P<0.05)。[结论]清热泄浊化瘀汤治疗湿热瘀阻型痛风疗效显著,值得推广。

[Objective]To observe the heat leakage of turbid blood decoction in the treatment of dampness heat and blood stasis resistance effect of gout.[Methods]A retrospective analysis of the proposed heat discharge turbid blood soup(Tufuling,Weilingxian20g,Baishu,Danggui,Dangshen,Shenghuangshi,Fabanxia,Chenpi,Dahuang,Zexie,Rendongteng,Jixueteng,Sheshecao,Shancigu,Cheqianzi,Shiwei,Shengyiren,Honghua 10g each.Acute and Zhimu,Mangxiao,Shengdihuang,Leigongteng 10g;gout remissionXianmao 10g,Huaishanyao 30g,Buguzhi 20g;tophus and Chuanshanjia,Chishao 10g,40g Jinqiancao,Wugong 3g).1/d,boiling water 300 ml,oral sooner or later.Continuous treatment of 14d to 1 course of treatment.Observation of clinical symptoms,blood uric acid concentration,adverse reactions.Continuous treatment 2 period of treatment(28d),to determine efficacy.[Results]of 34 cases,13 cases were markedly effective,effective 18 cases,3 had no effect,the total effective rate was 91.17%.Blood uric acid index data differences significant before and after the treatment(P<0.05).[Conclusion]Heat discharge turbid blood decoction has significant effect for dampness heat and blood stasis resistance gout,is worth promoting.
