针灸与益气活血汤联合甲基强的松龙治疗急性脊髓损伤随机平行对照研究

Acupuncture and Moxibustion with Yiqi Huoxue Soup Methyl Prednisolone The Treatment of Acute Spinal Cord Injury Random Parallel Control Study

[目的]观察针灸与益气活血汤联合甲基强的松龙治疗急性脊髓损伤疗效。[方法]使用随机平行对照方法,将80例住院患者按住院顺序随机分为两组。对照组40例甲基强的松龙,开始15～20mg/d,早晚2次服,维持量4～8mg/d。治疗组40例针灸:取后溪、申脉、督脉穴、大椎、腰俞、交感、脑点、神门、皮质下为主穴,坐骨神经及臀神经为辅穴,同时以耳背、耳轮等脊髓相关穴位作为针灸部位,1次/d,根据患者病症及年龄、病史特点将针灸时间控制在25～60min内,持续针灸治疗直至病症得以控制。益气活血汤(黄芪28～33g,红花8～12g,三七粉4～7g,水蛭8～11g,肉桂6g,丹参31～33g,桃仁7～12g),1剂/d,水煎500mL,早晚口服。西药治疗同对照组。连续治疗20d为1疗程。观测临床症状、触觉、痛觉、运动、括约肌功能、运动功能、自理能力、不良反应。连续治疗3疗程,判定疗效。[结果]治疗组显效23例,有效8例,无效9例,总有效率77.50%。对照组显效8例,有效14例,无效18例,总有效率55.00%。治疗组疗效优于对照组(P<0.05)。神经及运动功能改善治疗组优于对照组(P<0.05)。[结论]针灸与益气活血汤联合甲基强的松龙治疗急性脊髓损伤效果显著,值得推广。

[Objective]To observe the effect of Yiqi Huoxue Decoction Combined with acupuncture and conventional conservative treatment efficacy in the treatment of acute spinal cord injury.[Methods]using randomized controlled Methods,80 patients were randomly divided into two groups.40 cases of the control group conventional conservative treatment.The treatment group of 40 cases of acupuncture and Moxibustion: after the river,Shen Mai,Du meridian,Dazhui,Yaoshu,sympathetic,brain,God door,subcortical main points,sciatic nerve and gluteal nerve as points,at the same time,such as ear helix spinal cord related points as acupuncture sites,1 /d,according to the patient's symptoms and age the characteristics of acupuncture and moxibustion time,history will be controlled within 25 ～ 60min,continued acupuncture treatment until the disease under control.Prescription:Astragalus Yiqi Huoxue Decoction(Huangshi28 ～ 33g,Honghua8 ～ 12g,Sanqifen4 ～ 7g,Shuizhi8 ～ 11g,Rougui6g,Danshen 31 ～ 33g,Taoren7 ～ 12g)agent/ d,Decoction 500mL,sooner or later oral.The two groups were continuous treatment with 20d for 1 courses.Observation of clinical symptoms,tactile,pain,movement,sphincter function,motor function,ADL,adverse reaction.Continuous 3 courses of treatment,the curative effect.23 cases of group therapy[Results]effective 8 cases,invalid 9 cases,the total efficiency of 77.50%.The control group 8 cases markedly effective,effective 14 cases,invalid 18 cases,the total efficiency of 55%.The efficacy of the treatment group than in control group(P<0.05).The control group and the treatment group was better than the nerve functional status(P<0.05).[Conclusion]the acupuncture treatment and Yiqi Huoxue Decoction Combined with conventional conservative treatment of acute spinal cord injury effect is significant,worthy of promotion.
