循经取穴针刺联合生物电反馈、吞咽训练治疗脑卒中后吞咽障碍随机平行对照研究

Acupoints along Meridians Acupuncture and Bio-electrical Feedback,Swallowing Training in Treatment of Dysphagia after Stroke Randomized Controlled Study

[目的]观察循经取穴针刺联合生物电反馈、吞咽训练治疗脑卒中后吞咽障碍疗效。[方法]使用随机平行对照方法,将35例住院患者按随机数字表法分为两组。对照组17例根据病情,对患者脑细胞进行保护和改善循环等常规治疗。多潘立酮、莫沙必利、伊托必利等促胃肠动力剂;胃黏膜保护剂(铋制剂、铝碳酸镁、复方三硅酸镁或硫糖铝等);同时进行必要的心理安慰、鼓励及支持,及时心理沟通和疏导,尽快消除患者心理恐惧和焦虑等情绪。治疗组18例辨证循经取穴针刺+肌电生物反馈疗法+吞咽训练等综合治疗。两组均连续治疗14d为1疗程。观测临床症状、VFSS评分、不良反应。连续治疗4疗程,判定疗效。[结果]治疗组痊愈8例,显效5例,有效3例,无效2例,总有效率88.90%。对照组痊愈5例,显效2例,有效3例,无效7例,总有效率58.80%。治疗组疗效优于对照组(P<0.05)。VFSS评分两组均有改善,治疗组改善优于对照组(P<0.05)。[结论]循经取穴针刺联合生物电反馈、吞咽训练治疗脑卒中后吞咽障碍效果显著,值得推广使用。

[Objective]Observed acupoints along meridians acupuncture combined bioelectric feedback,swallowing training in treatment of dysphagia after stroke effect.[Methods]Using randomized controlled method,35 cases of hospitalized patients were randomly France into two groups.The control group of 17 patients according to the disease,the patient is protected brain cells and improve circulation and other conventional treatment,The use of domperidone,mosapride,itopride other prokinetic agents and mucosal protective agents(bismuth preparations,aluminum,magnesium carbonate,magnesium trisilicate compound or sucralfate,etc.)for treatment.While making necessary psychological comfort and encouragement and support,timely communication and psychological counseling,mental patients as quickly as possible to eliminate fear and anxiety and other emotions.18 cases of acupuncture treatment group + the electromyographic biofeedback therapy + swallowing training and other comprehensive treatment.14d continuous treatment in both groups as a course of treatment,observation of clinical symptoms,VFSS score,adverse reactions.Four cycles of continuous treatment,to determine efficacy[.Results]The cure in 8 cases,5 cases markedly effective in 3 cases,2 cases,the total efficiency of 88.90%.Control group cured in 5 cases,2 cases markedly effective in 3 cases,seven cases,the total efficiency 58.80%.Treatment group than the control group(P<0.05).VFSS score improved in both groups,the treatment group than the control group improved(P<0.05).[Conclusion]Acupoints along meridians acupuncture combined bioelectric feedback,swallowing training in treatment of dysphagia after stroke effect is significant and should be introduced.
