越婢加半夏汤联合西药治疗慢性阻塞性肺疾病急性加重期随机平行对照研究

Modified Yuebi Decoction Plus Pinellia Decoction Combined with Western Medicine in Treatment of Acute Exacerbations of Chronic Obstructive Blocking Randomized Study of Lung Disease

[目的]观察越婢加半夏汤联合西医治疗慢性阻塞性肺疾病急性加重期疗效。[方法]使用随机平行对照方法,将80例门诊及住院患者按抽签法随机分为两组。对照组40例常规治疗:低流量吸氧,电解质、水、酸碱平衡维持等对症性治疗。平喘解痉,氨茶碱,0.1g/次,3次/d,口服;生理盐水250mL/次,1次/d,静滴。抗感染,头孢哌酮舒巴坦2.0g/次,1次/d,静注;生理盐水(100mL/次,1次/d,静滴。去痰,氨溴索,30mg/次,3次/d,口服。治疗组40例越婢加半夏汤(麻黄10g,石膏煅捣、牛蒡子炒,捣、清半夏、元参各15g,生山药25g,寸麦冬带心20g,甘草7.5g,大枣3枚,生姜3片。痰黏稠不易咳出、胸闷气逆、痰热内盛,加鱼腥草、瓜蒌各20g;喘息不得平卧与痰鸣,加射干、葶苈子各10g;口干舌燥、痰热伤津,加芦根20g,天花粉15g,大便秘结、腹气不通,加大黄10g)。1剂/d,水煎150mL,早晚口服。西药治疗同对照组。连续治疗15d为1疗程。观测临床症状、不良反应。治疗1疗程,判定疗效。[结果]治疗组显效12例,有效23例,无效5例,总有效率87.50%。对照组显效6例,有效24例,无效10例,总有效率75.00%。治疗组疗效优于对照组(P<0.05)。[结论]越婢加半夏汤联合西医治疗慢性阻塞性肺疾病急性加重期效果显著,值得推广。

The treatment group of 40 cases and modified yuebi Decoction plus Pinellia(Mahuang10g,Shigao calcined tamping,Niupanzi fried,Qingbanxia,Yuanshen each 15g,Shengshanyao,Cunmaidong(hearts)each 20g,Gancao 7.5g,Dazao 3 pieces,3 pieces of Shenjiang. Sticky sputum,chest tightness is not easy to cough,phlegm heat Qi Sheng,adding Yuxingcao,Gualou each 20g;wheezing shall lie with phlegm,plus Shegan,Tinglizi each 10g;mouth parched and tongue scorched,phlegm heat injury Tianjin,with Lugen 20g,15g Tianhuafen,constipation,abdominal gas barrier,plus Dahuang 10g). 1 doses of /d,water simmer in water 150mL,sooner or later oral. Western medicine treatment with the control group.Continuous treatment with 15d for 1 courses. Observation of clinical symptoms,adverse reaction. 1 courses of treatment,the curative effect. 12 cases of group therapy[Results],23 cases were effective,5 cases ineffective,the total efficiency of 87.50%. 6 cases of control group,24 cases effective,10 cases ineffective,the total efficiency of 75%. The treatment group was better than control group(P< 0.05)[.Conclusion]modified yuebi Decoction plus Pinellia decoction combined with western medicine in treatment of chronic obstructive pulmonary disease in acute exacerbation period effect is significant,worthy of promotion.
