超关节推拿联合针刺治疗桡骨茎突狭窄性腱鞘炎随机平行对照研究

Super Joints Massage Combined Acupuncture in Treatment of Radial Styloid Process Narrow Tenosynovitis Random Parallel Control Study

[目的]观察超关节推拿联合针刺治疗桡骨茎突狭窄性腱鞘炎疗效。[方法]使用随机平行对照方法,将92例门诊患者按抽签法随机分为两组。对照组44例针刺,活血止痛。取穴:患侧曲池、偏历、温溜、阳溪、列缺、合谷、鱼际诸穴。用0.30mm×40mm针灸针,施以平补平泻手法。留针20min,1次/d。治疗组48例超关节推拿法,活血舒筋,消肿止痛。以拇指按揉在肱骨外上髁处,约2min;而后以弹拨及一指禅推法沿前臂桡侧至腕关节,点按曲池、偏历、温溜、阳溪、列缺、合谷、鱼际等穴,每穴操作1min。桡骨茎突部及其上下方或腕关节疼痛处继用拇指颤法治疗约2min;自拇指、食指、中指掌指关节经腕关节推抹至患侧肘部,配合轻巧搓揉法,轻度拔伸下将患手缓缓旋转及屈伸治疗约5min。上述手法均1次/d。针刺治疗同对照组。连续治疗12d为1疗程。观测临床症状、腕关节活动度、不良反应。治疗1疗程,判定疗效。[结果]治疗组痊愈41例,好转5例,未愈2例,总有效率95.83%。对照组痊愈25例,好转8例,未愈11例,总有效率75.00%。治疗组疗效优于对照组(P<0.05)。腕关节活动度两组均有改善(P<0.05),治疗组改善优于对照组(P<0.05)。[结论]超关节推拿联合针刺治疗桡骨茎突狭窄性腱鞘炎效果显著,值得推广。
