活血汤联合西药治疗类风湿关节炎随机平行对照研究

A Randomized Controlled Trial of Huoxue Decoction Combined with Western Medicine in the Treatment of Rheumatoid Arthritis

[目的]观察活血汤联合西药治疗类风湿关节炎疗效。[方法]使用随机平行对照方法,将50例门诊患者按随机数字表法分为两组。对照组25例糖皮质激素,醋酸泼尼松片,10mg/次,1次/d,口服。双氯芬酸缓释胶囊,25mg/次,3次/d,口服。细胞毒性药,甲氨蝶呤(MTX),2.5mg/次,1次/7d,口服。耐受加强及体征逐渐缓解时减少前二者药物使用量,甲氨蝶呤则保持1次/7d。治疗组25例活血汤(水蛭5g,红花、僵蚕、全蝎、土鳖虫各10g,海风藤、骨碎补各15g,土茯苓、山慈菇、半边莲、白花蛇舌草、淮牛膝各20g,蜈蚣2条),1剂/d,水煎200mL,早晚口服。西药治疗同对照组。连续治疗30d为1疗程。观测临床症状、不良反应。连续治疗2疗程,判定疗效。随访60d,观测复发率。[结果]治疗组痊愈10例,有效13例,无效2例,总有效率92.00%。对照组痊愈7例,有效12例,无效6例,总有效率76.00%。治疗组疗效优于对照组(P<0.05)。[结论]活血汤联合西药治疗类风湿关节炎效果显著,值得推广。

[Objective]To observe the therapeutic effect of Huoxue Decoction Combined with western medicine in the treatment of rheumatoid arthritis. [Methods] Using randomized controlled methods, 50 patients were randomly divided into two groups. 25 cases of the control group of glucocorticoid, Prednisone Acetate Tablets, 10mg/, 1 /d, oral. Diclofenac sustained-release capsules, 25mg/ times, 3 /d, oral. Cytotoxic drugs, methotrexate(MTX), 2.5mg/, 1 /7d, oral. The former two drug use decreased tolerance to strengthen and signs gradually eased, but methotrexate insisted on 1 /7d. The treatment group of 25 cases of Huoxue Decoction(Shuizhi 5g,Honghua,Jiangcan,Quanxie,Tubiechong each 10g,Haifengteng,Gusuibu each 15g, Tufuling,Shancigu,Banbianlian,Baihuashecao,Huainiuxi each 20g,Wugong 2 slip), 1 doses of /d, water simmer in water 200mL, sooner or later oral. Western medicine treatment with the control group. Continuous treatment with 30d for 1 courses. Observation of clinical symptoms, adverse reaction. Continuous 2 courses of treatment, the curative effect. Follow up 60d, observing the recurrence rate. [Results] The treatment group cured 10 cases, effective 13 cases, invalid 2 cases, the total efficiency of 92%. The control group cured 7 cases, effective 12 cases, invalid 6 cases, the total efficiency of 76%. The treatment group was better than control group(P< 0.05). [Conclusion] Huoxue Decoction Combined with western medicine has remarkable effect for the treatment of rheumatoid arthritis, worthy of promotion.
