活血解毒通络汤联合甲钴胺治疗糖尿病周围神经病变(瘀血阻络)随机平行对照研究

Randomized Parallel Controlled Study of Huoxue Jiedu Tongluo Decoction(活血解毒通络汤) combined with Mecobalamin in the Treatment of Diabetic Peripheral Neuropathy(Yuxue Zuluo/瘀血阻络)

[目的]观察活血解毒通络汤联合甲钴胺治疗糖尿病周围神经病变(瘀血阻络)疗效。[方法]使用随机平行对照方法,将100例门诊患者按就诊顺序号随机分为两组。对照组50例甲钴胺,0.5/次,3次/d。治疗组50例活血解毒通络(黄芪30g,川芎、当归各10g,鸡血藤15g,苍术、玄参各10g,白芍、桂枝各15g,细辛10g),水煎250mL,早晚温服,1剂/d;甲钴胺治疗同对照组。连续治疗20d为1疗程。观测临床表现、症状评分(肢体麻木、肢末刺痛、乏力、气虚少言、头晕、畏风)、不良反应。治疗1疗程(20d),判定疗效。[结果]治疗组显效30例,有效14例,无效6例,总有效率88.00%;对照组显效20例,有效15例,无效15例,总有效率70.00%;治疗组疗效优于对照组(P<0.05)。肢体麻木、肢末刺痛、乏力、气虚少言、头晕、畏风、两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01)。[结论]活血解毒通络汤联合甲钴胺治疗糖尿病周围神经病变(瘀血阻络),疗效满意,无严重不良反应,值得推广。

[Objective]To observe the therapeutic effect of Huoxue Jiedu Tongluo decoction combined with mecobalamin on diabetic peripheral neuropathy(Yuxue Zuluo).[Method]100 out-patients were divided into 2 groups according to the order of visiting.50 cases of the control group,mecobalamin,0.5 times,3 times/d.50 cases of the treatment group,were treated by Huoxue Jiedu Tongluo decoction(Huangqi30 g,Chuanxiong,Danggui each 10 g,Jixueteng 15 g,Cangzhu,Xuanshen each 10 g,Baishao,Guizhi each15 g,Xixin 10 g),decocted by water 250 mL,taken warmly in the morning and evening,one dose per day;methyl continuous treatment for 20 days is a course.Clinical manifestations,symptom scores and adverse reactions were observed.One course(20 days)was used to determine the curative effect.[Result]The total effective rate was 88.00%in the treatment group,the total effective rate was 70.00%;the curative effect of the treatment group was better the control group(P<0.05).The numbness of limbs,tingling pain at the end of limbs,fatigue,deficiency of Qi,less speech,dizziness,fear of wind and the two groups were all improved(P<0.01),and the effective in the treatment group was better than the control group(P<0.01).[Conclusion]Huoxue Jiedu Tongluo decoction combined with mecobalamin in the treatment of diabetic peripheral neuropathy(Yuxue Zuluo)has satisfactory curative effect and no serious adverse reactions,which is worthy of promotion.
