妇科消囊汤联合西药治疗卵巢囊肿随机平行对照研究

Randomized Parallel Controlled Study on the Treatment of Ovarian Cyst with Fuke Xiaonang Decoction(妇科消囊汤) and Western Medicine

[目的]观察妇科消囊汤联合西药治疗卵巢囊肿疗效。[方法]使用随机平行对照方法,将70例门诊患者按就诊顺序号抽签随机分为两组。对照组30例头孢呋辛1.5g、奥硝唑0.5g+生理盐水250mL,静滴,2次/d。治疗组40例妇科消囊汤(鹿角霜、黄芪、白术、茯苓、菟丝子各18g,巴戟天、桂枝、丹参、鱼腥草各13g,甘草炙8g),水煎300mL,100mL/次,早晚分服;100mL加热至38℃,睡前排便后,灌肠袋衔接一次性肛管,石蜡油润滑后经由肛门插进直肠约7cm,慢慢灌入直肠,保留到第二天早晨,经期停用;西药治疗同对照组。连续治疗2个月为1疗程。观测临床表现、卵巢囊肿直径、血清雌二醇、孕酮、尿促卵泡素、促黄体生成素、不良反应。治疗1疗程(2个月),判定疗效。[结果]治疗组痊愈32例,有效6例,无效2例,总有效率95.00%;对照组痊愈15例,有效7例,无效8例,总有效率73.33%;治疗组疗效优于对照组(P<0.05)。卵巢囊肿直径、E_2、P、FSH、LH两组均有改善(P<0.05,P<0.01),治疗组改善优于对照组(P<0.01)。[结论]妇科消囊汤联合西药治疗卵巢囊肿,疗效满意,无严重不良反应,值得推广。

[Objective]To observe the curative effect of Fuke Xiaonang decoction combined with western medicine on ovarian cyst.[Method]Seventy outpatients were randomly divided into two groups by drawing lots according to the order of visiting.Control group 30 cases of cefuroxime 1.5 g,ornidazole 0.5 g+saline250 mL,intravenous drip,2 times/d.In the treatment group,40 cases of gynecological Fuke Xiaonang decoction(Lujiaoshuang,Huangqi,Baizhu,Fuling,Tusizi each 18 g,Bajitian,Guizhi,Danshen,Yuxingcao each 13 g Zhigancao 8 g)were decocted with water 300 mL,100 mL/time,and divided into two groups in the morning and evening;100 mL was heated to 38 C,before and after defecation,the enema bag was connected with one-time anal canal,and then lubricated with paraffin oil,it was inserted into the rectum slowly,Two days in the morning,the menstrual period was discontinued;western medicine treatment was the same as the control group.Continuous treatment for 2 months is a course of treatment.Clinical manifestations,ovarian cyst diameter,serum estradiol,progesterone,urinary follicle stimulating hormone,luteinizing hormone and adverse reactions were observed.One course of treatment(2 months)was used to determine the curative effect.[Result]In the treatment group,32 cases were cured,6 cases were effective,2 cases were ineffective,the total effective rate was 95.00%.In the control group,15 cases were cured,7 cases were effective and8 cases were ineffective,the total effective rate was 73.33%.The curative effect of the treatment group was better than that of the control group(P<0.05).The diameter of ovarian cyst,E2,P,FSH and LH were improved in both groups(P<0.05,P<0.01).The improvement in the treatment group was better than that in the control group(P<0.01).[Conclusion]Fuke Xiaonang decoction combined with western medicine in the treatment of ovarian cyst has satisfactory curative effect and no serious adverse reactions,which is worthy of promotion.
