癌症疾病进展患者焦虑和抑郁影响因素分析

Analysis of the influencing factors of anxiety and depression in patients with cancer progression

目的分析癌症疾病进展患者焦虑和抑郁发生的影响因素。方法选取122例癌症疾病进展患者使用焦虑自评量表(self-rating anxiety scale,SAS)及抑郁自评量表(self-rating depression scale,SDS)进行焦虑和抑郁状态的初步筛查,并分析其影响因素。结果 122例癌症疾病进展患者中存在焦虑状态94例(77.0%),抑郁状态98例(80.3%),均以轻、中度为主。SAS评分为(58.11±10.25)分,SDS评分为(60.48±10.20)分。多因素分析显示,女性、初中以下文化程度及乳腺、妇科、消化道肿瘤的患者易出现焦虑和抑郁状态(均P<0.05),中老年患者易出现抑郁状态(P<0.05)。结论焦虑和抑郁在癌症疾病进展患者中发生率均较高,在抗癌治疗及临床心理筛查中应密切关注性别、年龄、文化程度及肿瘤类型对焦虑和抑郁产生的影响。

Objective To analyze the influencing factors of anxiety and depression in patients with cancer progression.Methods A total of 122 cancer patients with disease progression were initially screened for the status of anxiety and depression,using self-rating anxiety scale(SAS)and self-rating depression scale(SDS),and the influencing factors were then analyzed.Results Among the 122 cases of patients with cancer progression,94 cases showed signs of anxiety(77.0%),and 98 cases were at the status of depression(80.3%).The psychological signs in all patients were mild or moderate.The SAS score was(58.11±10.25)and the SDS score was(60.48±10.20)points.Multivariate analysis showed that female,educational level below junior high school,and tumors in breast,gynecological system or gastrointestinal system were all risk factors of anxiety and depression(all P<0.05).Moreover,middle-aged and elderly patients were prone to suffer from depression(P<0.05).Conclusion The incidence of anxiety and depression is high in patients with cancer disease progression.During anticancer therapy and clinical psychological screening,close attention should be paid to the influencing factors of anxiety and depression,including gender,age,educational level and tumor type.
