小剂量丙泊酚复合腰丛联合坐骨神经阻滞对高龄股骨粗隆间骨折手术患者MAP、HR及OAA/S评分的影响 被引量:7

摘要 目的探讨小剂量丙泊酚复合腰丛联合坐骨神经阻滞对高龄股骨粗隆间骨折手术患者MAP、HR及OAA/S评分的影响。方法选择2016年3月—2017年5月在商丘市第一人民医院手术部麻醉科行股骨粗隆间骨折手术50例患者随机分为25例对照组(静脉全身麻醉)与25例观察组(小剂量丙泊酚复合腰丛联合坐骨神经阻滞)。比较患者不同时间点循环参数和警觉/镇静(OAA/S)评分。结果观察组T_2的MAP、HR水平均高于对照组,差异有统计学意义(P<0.05);观察组T_3的OAA/S评分高于对照组,差异有统计学意义(P<0.05)。结论小剂量丙泊酚复合腰丛联合坐骨神经阻滞对高龄股骨粗隆间骨折手术患者的影响较小且麻醉效果显著。 Objective To investigate the effect of low-dose propofol combined with lumbar plexus-sciatic nerve block on MAP, HR and OAA/S scores in patients with senile intertrochanteric fracture. Methods 50 patients with intertrochanteric fracture surgery in operation department of Shangqiu first people's hospital from March 2016 to May 2017 were selected, and they were randomly divided into 25 cases of control group(intravenous general anaesthesia) and 25 cases of observation group(low-dose propofol combined with lumbar plexus-sciatic nerve block). The loop parameter and the Observer's Assessment of Alertness/Sedation Scale(OAA/S) at different time were compared. Results The levels of MAP and HR during T_2 in observation group were higher than those in control group(P<0.05); The OAA/S score in T3 in observation group was higher than that in control group(P<0.05). Conclusion Low-dose propofol combined with lumbar plexus-sciatic nerve block has less effect on patients with senile intertrochanteric fracture, and it has significant anesthetic effects.
