老年患者下肢深静脉血栓的超声诊断价值

Elderly Patients with Lower Extremity Deep Vein Thrombosis of the Ultrasonic Diagnosis Value

目的观察超声诊断在老年患者下肢深静脉血栓中的应用,评价其临床价值。方法以2007年1月—2017年1月为研究时间段,将期间该院确诊的下肢深静脉血栓200例老年患者(年龄≥60岁)为研究对象,该组患者均予以超声检查,将其检查结果予以病理学诊断结果展开对比分析,总结超声检查在诊断老年下肢深静脉血栓中的效果。结果该组患者中共计患肢213条,检测显示其中表现为左下肢血栓135条,血栓部位共计245个,右下肢血栓78条,血栓部位共计182个;另外急性期血栓共计158条,超声表现为低回声,患处静脉管径呈明显增宽,阻塞处表现为血流充盈缺损样;亚急性期血栓55条,超声显示表现强回声、偏低回声、不均质回声等,血管管径则显示粗细不均匀,管壁呈增厚、粗糙样,血栓处血流充盈出现缺损;另外经统计,超声对病情检出率为96.00%(192/200),与病理学诊断100.00%比较差异无统计学意义(P>0.05);该组患者发病部位中以胫后静脉54.00%(108/200)占比率最高,其他依次为前静脉19.00%(38/200)、腘静脉11.00%(22/200)、股浅静脉9.50%(19/200)、骨深静脉6.50%(13/200)。结论超声诊断应用于老年患者下肢深静脉血栓中效果确切,有助于早期发现患者下肢深静脉血栓程度、血栓部位、血流表现等情况,以此为临床诊断与治疗提供数据支持,具有临床应用与推广价值。

Objective to observe the ultrasonic diagnosis in elderly patients with lower extremity deep vein thrombosis,the application of evaluation of its clinical value. Methods From,January 2007 to January 2017, will the author's hospital diagnosed during the period of lower extremity deep vein thrombosis, 200 cases of elderly patients(age 60 years or more)as the research object, the patients were ultrasound examination, the test results shall be pathology diagnosis contrast analysis, summary of ultrasound in the diagnosis of senile effect of deep vein thrombosis of lower limbs. Results This group of patients with limb 213, tests revealed the performance for the left leg thrombosis 135, thrombosis area a total of245, article 78 the right lower limb thrombosis and thrombosis area a total of 182; Article and a total of 158 acute thrombosis,The ultrasonography showed a low echo, and the diameter of the vein in the affected area was significantly increased, and the obstruction was manifested as blood filling defect;Subacute thrombosis in article 55, ultrasound shows strong echoes, low echo, heterogeneity echo, blood vessel diameter showed uneven thickness, tube wall thickening, rough sample, blood clots in the blood filling defect;In addition, the detection rate of ultrasound was 96.00%(192/200),There was no significant difference with the pathological diagnosis of 100.00%(P>0.05);The incidence site of this group was the highest ratio of 54.00%(108/200),The other sequence was 19.00%(38/200), popliteal vein 11.00%(22/200), superficial vein 9.50%(19/200), and bone deep vein 6.50%(13/200). Conclusion Ultrasound diagnosis is applied to elderly patients with lower extremity deep vein thrombosis in effect is exact, help to early detection of patients with lower limb deep vein thrombosis, thrombus parts and performances of the blood flow to provide data support for clinical diagnosis and treatment, clinical application and popularization value.
