膝关节退行性骨关节病影像学分型与钙化层组织病理改变的关系 被引量:1

Between Imaging Classification and Pathrelationshipological Changes of Calcified Layer in Degenerative Osteoarthropathy of Knee Joint

摘要 目的探究膝关节退行性骨关节病影像学分型与钙化层组织病理改变的关系。方法选取膝关节退行性骨关节病患者50例为该研究对象,时间选取为2014年1月—2018年1月,其均接受膝关节置换术治疗,在患者接受手术前,均为其实施X线以及MRI检查,根据其病变的部位,将病变分为髌股关节型(1型)11例,颈股关节型(2型)12例,髁间型(3型)9例,混合型(4型)18例,在其术后,实施切片病理检查,对其钙化层的病理变化情况进行观察,并分析其与影像学分型之间的关系。结果影像学分型为髌股关节型(1型)的膝关节退行性骨关节病患者,其主要的病理类型为A型、C型;颈股关节型(2型)患者,其主要的病理类型为A型、B型;髁间型(3型)患者,其主要的病理类型为A型;混合型(4型)患者,其主要的病理类型为A型、C型。结论膝关节退行性骨关节病患者其股骨髁负重区的相关钙化层存在形态学改变的情况,不同的影像学分型,其钙化层的病理变化存在一定的差异性。 Objective The purpose of this study was to investigate the relationship between radiographic classification of knee degenerative osteoarthritis and pathological changes of calcified layer. Methods 50 cases of knee degenerative osteoarticular disease were selected as the subjects of this study. The time was selected from January 2014 to January2018. All patients were treated with knee arthroplasty. According to the site of the lesion, the lesions were divided into3 types: 11 cases of patellofemoral joint type(type 1), 12 cases of neck joint type(type 2), 9 cases of intercondylar type(type 3) and 18 cases of mixed type(type 4). The pathological changes of calcification layer were observed and the relationship between calcification and imaging classification was analyzed. Results The main pathological types of degenerative osteoarthropathy of knee joint were type A, type C, type 2, type A and type B, respectively. The main pathological type of the patients was type A and type 4(mixed type). The main pathological type of the patients was type A and type C. Conclusion In the patients with degenerative osteoarthropathy of the knee, there are morphological changes in the related calcification layer of the femur condylar bearing area, and different imaging credits. The pathological changes of calcification layer are different.
