老年股骨颈骨折手术患者腰硬联合麻醉和全身麻醉的效果比较

Comparison of the results of combined spinal and epidural anesthesia and general anesthesia in elderly patients with femoral neck fractures

目的分析老年股骨颈骨折手术患者腰硬联合麻醉和全身麻醉的效果。方法选择2017年4月—2018年4月该院收治的90例老年股骨颈骨折手术患者,随机分为对照组和观察组,对照组采用全身麻醉,观察组给予腰硬联合麻醉,比较两组的麻醉后的镇痛效果和不良反应情况。结果对照组的总有效率为91.11%,观察组总有效率95.54%,观察组略高于对照组,但差异无统计学意义(P>0.05);观察组不良反应发生率6.55%低于对照组22.91%(P<0.05)。结论老年股骨颈骨折手术治疗麻醉采用全身麻醉和腰硬联合麻醉均有良好的麻醉效果,但采用腰硬联合麻醉的不良反应发生率低,安全性高,具有实际应用价值,值得临床推广应用。

Objective To analyze the combined lumbar and anesthesia and general anesthesia in elderly patients with femoral neck fractures. Medhods A total of 90 elderly patients with femoral neck fractures treated in our hospital from April 2017 to April 2018 were randomly divided into a control group and an observation group. The control group was treated with general anesthesia. The observation group was given combined lumbar and anesthesia to compare the two groups. The analgesic effect and adverse reactions of the group after anesthesia. Results The total effective rate was 91.11% in the control group and 95.54% in the observation group. The observation group was slightly higher than the control group, but the difference was not statistically significant(P>0.05). The incidence of adverse reactions in the observation group was 6.55%. It was lower than the control group by 22.91%(P<0.05). Conclusion The surgical anesthesia for femoral neck fractures in the elderly has good anesthetic effect with general anesthesia and combined spinal and epidural anesthesia. However, the combined use of lumbar and hard anesthesia has low incidence of adverse reactions and high safety. It has practical value and is worthy of clinical application..
