不同比重布比卡因腰硬联合麻醉对剖宫产术后下肢运动功能的影响

Effect of Combined Spinal and Epidural Anesthesia with Bupivacaine of Different Specific Proportions on Lower Limb Motor Function After Cesarean Section

目的分析不同比重布比卡因腰硬联合麻醉对剖宫产术后下肢运动功能的影响。方法选取该院收治的择期行剖宫产术(115例)产妇为该次研究对象,依据随机原则将研究对象分为对照组(56例)和观察组(59例),观察组患者使用等比重麻药,麻醉方式为腰麻联合硬膜外麻醉,对照组患者使用重比重麻药,麻醉方式为腰麻联合硬膜外麻醉,对比分析两组产妇的下肢运动功能状况。结果两组患者均存在运动阻滞的情况,观察组患者下肢无可运动关节者占比比对照组少53.14%,踝关节及其以下部位可以运动,但是以上部位不可以运动者占比比对照组高36.20%,抬腿持续时间在10 s及以上者占比比对照组高16.95%,组间数据比较,差异有统计学意义(P<0.05)。结论针对剖宫产产妇进行麻醉的时候,使用等比重麻药进行腰麻和硬膜外麻醉处理,可以有效控制下肢运动阻滞的发展程度,具有较高的安全性。

Objective To analyze the effect of combined spinal anesthesia with bupivacaine with different proportions on lower extremity motor function after cesarean section. Methods According to the random principle, the subjects were divided into two groups: the control group(56 cases) and the observation group(59 cases). The patients in the observation group were treated with the same proportion of anesthetics, the anesthesia was combined with spinal anesthesia and epidural anesthesia, and the patients in the control group were treated with heavy specific gravity anesthesia. The mode of anesthesia was spinal anesthesia combined with epidural anesthesia. The motor function of lower extremities of the two groups was compared and analyzed. Results There was motor block in both groups, and the patients in the observation group were treated with motor block. 53.14% less than the control group, and the ankle joint and the following parts can be moved. However, the proportion of those who could not exercise above was 36.20% higher than that of the control group, and the proportion of those with leg lifting duration of 10 s or more was 16.95%.Compared with the control group,there was a statistical difference between the two groups(P <0.05). Conclusion When the cesarean section women are anesthetized, The development of lower extremity motor block can be effectively controlled by the use of equal proportion anesthetic for spinal and epidural anesthesia.
