老年患者下肢骨科手术后进行静脉镇痛对患者术后恢复的临床研究

Clinical Study of Postoperative Intravenous Analgesia for Elderly Patients after Operation in Lower Limb Department of Orthopedics

目的探讨老年患者下肢骨科手术后进行静脉镇痛对患者术后恢复的临床研究。方法选取2017年5月—2018年5月收治的40例行下肢骨科手术的老年患者,并将之分为对照组与观察组,对照组手术后未进行静脉镇痛,观察组在手术结束后常规进行静脉术后镇痛,两组均辅以手法按摩,对两组术后下肢静脉血栓发生率、术后恢复满意度以及术后平均住院时间进行对照分析。结果下肢深静脉血栓发生率:观察组6.7%,对照组26.7%,差异有统计学意义(P<0.05);术后恢复满意度:观察组93.3%,对照组73.3%,差异有统计学意义(P<0.05);术后平均住院时间:观察组8～10 d,对照组10～14 d,差异有统计学意义(P<0.05)。结论因此老年患下肢骨科手术后早期静脉镇痛有利于预防患者下肢深静脉血栓形成,能够提升患者术后恢复满意度,可明显缩短患者平均住院天数,有明显的临床应用价值,值得推广。

Objective To investigate the effect of intravenous analgesia on postoperative recovery of elderly patients after orthopaedic surgery of lower limbs. Methods 40 elderly patients who underwent orthopaedic surgery of lower extremity from May 2017 to May 2018 were divided into control group and observation group. No intravenous analgesia was performed in the control group after operation. The observation group was given intravenous analgesia routinely after operation. Both groups were assisted by manipulative massage to treat venous thrombosis of lower extremity after operation.The incidence, postoperative satisfaction and postoperative hospital stay were analyzed. Results The incidence of DVT was 6.7% in the observation group, 26.7% in the control group,the difference was statistically significant(P<0.05);the postoperative recovery satisfaction was 93.3% in the observation group and 73.3% in the control group, the difference was statistically significant(P<0.05);the average postoperative hospital stay was 8-10 days in the observation group and 10-14 days in the control group, the difference was statistically significant(P<0.05). Conclusion Early intravenous analgesia after orthopaedic surgery in elderly patients with lower extremity is helpful to prevent the formation of deep vein thrombosis in patients with lower extremity, can improve postoperative recovery satisfaction, can significantly shorten the average hospitalization days of patients, has obvious clinical application value, is worth promoting.
