小剂量轻比重腰麻-硬膜外联合麻醉在老年下肢骨折手术应用效果观察

Observation on the Effect of Low-dose Light-weight Spinal Anesthesia Combined with Epidural Anesthesia in Elderly Patients with Lower Extremity Fracture

目的观察小剂量轻比重腰麻-硬膜外联合麻醉在老年下肢骨折手术应用效果。方法选取2017年5月—2018年5月该院治疗的老年下肢骨折需手术患者110例,随机分两组,55例对照组单独硬膜外麻醉,55例研究组小剂量轻比重腰麻-硬膜外联合麻醉,比较两组麻醉各项指标及不良反应发生率。结果研究组麻醉量(6.23±0.92)mg、阻滞起效时间(5.21±2.43)s、完善时间(6.93±3.12)s均比对照组少,比较差异有统计学意义(P<0.05),且研究组不良反应发生率1.82%比对照组低,比较差异有统计学意义(P<0.05)。结论小剂量轻比重腰麻-硬膜外联合麻醉可提高老年下肢骨折手术效果,减少其不良反应发生率,具高安全性,临床应用具一定应用价值。

Objective To observe the effect of low-dose light-weight spinal anesthesia combined with epidural anesthesia in elderly patients with lower extremity fractures. Methods A total of 110 patients with lower extremity fractures treated in our hospital from May 2017 to May 2018 were randomly divided into two groups.Fifty-five patients underwent epidural anesthesia alone, and 55 patients in the study group underwent low-dose light-weight spinal anesthesia combined with epidural anesthesia. The anesthesia indicators and adverse reactions were compared between the two groups. Results The anesthetic volume(6.23±0.92) mg, blockade onset time(5.21±2.43) s, and perfection time(6.93±3.12) s were all less than the control group, and the difference was statistically significant(P<0.05). The incidence of adverse reactions in the study group was 1.82% lower than that in the control group, and the difference was statistically significant(P<0.05). Conclusion Low-dose light-weight lumbar anesthesia combined with epidural anesthesia can improve the surgical outcome of lower extremity fractures in the elderly, reduce the incidence of adverse reactions, and has high safety. The clinical application has certain application value.
