彩超对下肢骨折术前深静脉血栓筛查的意义

Significance of Color Doppler Ultrasound in Screening Deep Venous Thrombosis before Operation for Fracture of Lower Limbs

目的分析于下肢骨折手术前应用彩超进行深静脉血栓筛查的临床价值。方法以该院2017年6月—2018年6月诊治的行手术治疗的下肢骨折患者180例作为此次的研究样本,患者均于术前接受彩超检查。结果该组180例下肢骨折患者经由术前彩超筛查结果提示发生静脉血栓患者共计12例;深静脉血栓发生率为6.67%;其中左下肢静脉血栓患者6例,占比为3.33%;右下肢静脉血栓患者4例,占比为2.22%;双下肢静脉血栓者2例,占比为1.11%;在男女性别发生率比较中并差异无统计学意义(P>0.05);年龄≥50岁患者的发生率为9.18%较<50岁者的3.66%显著更高,差异有统计学意义(P<0.05);多发性骨折患者的深静脉血栓率为11.54%,较单发性骨折患者的4.69%显著更高,差异有统计学意义(P<0.05)。结论下肢骨折患者发生深静脉血栓的风险较高,手术前通过彩超筛查深静脉血栓具有很大必要性,彩超能够更为安全且有效的为手术治疗方案提供参考。

Objective To analyze the clinical value of deep venous thrombosis screening by color Doppler ultrasound before lower extremity fracture surgery. Methods 180 patients with lower extremity fractures treated from June 2017 to June 2018 were examined by color ultrasound before operation. Results Among 180 patients with lower extremity fracture, 12 cases had venous thrombosis, 6.67% cases had deep vein thrombosis, 6 cases had left lower extremity vein thrombosis(3.33%), 12 cases had venous thrombosis before operation. There were 4 patients with right lower extremity venous thrombosis(2.22%). 2 cases of deep venous thrombosis of lower extremity, accounting for 1.11%,and there was no significant difference between male and female(P>0.05). The incidence of age ≥ 50 was 9.18% and 3.66% respectively,the different was statistically significant(P<0.05). The rate of deep venous thrombosis in patients with multiple fractures was 11.54%, which was significantly higher than that in patients with single fracture(4.69%), the different was statistically significant(P <0.05). Conclusion The risk of deep venous thrombosis in patients with lower extremity fracture is higher. It is necessary to screen deep venous thrombosis by color Doppler ultrasound before operation. It is more safe and effective to provide reference for surgical treatment.
