MSCT三维重建技术在胫骨平台骨折分型的临床应用价值 被引量:2

Clinical Application Value of MSCT Three-dimensionalreconstruction in Typing of Tibial Plateau Fractures

摘要 目的探讨多层螺旋CT(MSCT)三维(3D)重建技术在胫骨平台骨折分型中价值。方法选择该院2016年2月—2018年7月收治的82例经手术证实的胫骨平台骨折患者作为研究对象,所有患者术前均拍摄患侧膝关节DR正侧位片和MSCT 3D重建检查,并与手术结果对照,比较DR和MSCT 3D重建对胫骨平台骨折分型的准确性。结果 MSCT 3D重建对I-VI型胫骨平台骨折分型的准确率均高于DR,但二者比较差异无统计学意义(P>0.05)。MSCT 3D重建对胫骨平台骨折分型的总体准确率为95.12%,明显高于DR的81.71%,差异有统计学有(P<0.05)。MSCT 3D重建对髁间嵴和腓骨小头骨折的诊断率为96.34%和95.12%,明显高于明显高于DR的70.73%和64.63%,差异有统计学意义(P<0.05)。结论 MSCT 3D重建技术对胫骨平台骨折分型准确率明显优于DR,值得临床推广应用。 Objective To investigate the value of Multi-slice spiral CT(MSCT) three-dimensional(SD)reconstruction in typing of tibial plateau fractures. Methods From Febuery 2016 to July 2018, 82 cases of tibial plateau fractures confirmed by surgery were studied. All the patients were preoperatively photographed with positive and lateral DR of the affected side knee and MSCT 3D reconstruction, and compared with the results of surgery, to compare the clinical value of DR and MSCT 3D reconstruction in typing of tibial plateau fractures. Results The accuracy rate of MSCT 3D reconstruction for the typing of tibial plateau fractures of I-VI type was higher than that of DR, but the difference was not statistically significant(P >0.05). The overall accuracy rate of MSCT 3D reconstruction for fracture typing of tibial plateau was95.12 %, it was significantly higher than81.71 % of DR,the difference was statistically significant(P<0.05). The diagnostic rate of MSCT 3D reconstruction for intercondylar ridge and peroneal microcephalic fractures was 96.34% and95.12%, those were significantly higher than 70.73% and 64.63 of DR,the difference was statistically significant(P <0.05). Conclusion The typing accuracy rate of MSCT 3D reconstruction for tibial plateau fractures were significantly better than that of DR,which is worthy of clinical application.
