附加侧方钢板手术治疗下肢骨折行髓内钉内固定术后骨不连疗效果评价

Evaluation of the Effect of Lateral Plate Surgery on Nonunion after Intramedullary Nail Fixation for Lower Limb Fracture

目的分析评价附加侧方钢板手术治疗下肢骨折行髓内钉内固定术后骨不连的效果。方法该次采取随机分组法,将该院在2015年1月—2016年1月收治的58例下肢骨折行髓内钉内固定术后骨不连患者分成2组;其中,对照组29例采取更换髓内钉行翻修手术治疗,观察组采取附加侧方钢板手术治疗,进一步比较两组手术治疗效果。结果 (1)在手术时间上,观察组明显短于对照组,差异有统计学意义(P<0.05);在术中出血量上,观察组明显少于对照组,差异有统计学意义(P<0.05);在术中输血量上,观察组明显少于对照组,差异有统计学意义(P<0.05);在住院时间上,观察组明显短于对照组,差异有统计学意义(P<0.05)。(2)随访1年,膝关节功能优良率,观察组为86.21%,与对照组的62.07%比较明显更高,差异有统计学意义(P<0.05)。结论在临床中,针对下肢骨折行髓内钉固定术后骨不连患者,采取附加侧方钢板手术治疗疗效显著,因此具备推广及使用的价值。

Objective To analyze and evaluate the effect of lateral plate surgery on nonunion of lower limb fractures after intramedullary nailing.Methods Fifty-eight patients with nonunion after intramedullary nail fixation for lower limb fractures in our hospital from January 2015 to January 2016 were divided into two groups by random grouping method.Among them,29 patients in the control group were treated by replacing intramedullary nails and revision operation,while the observation group was treated by additional lateral plate operation,and the therapeutic effects of the two groups were further compared.Results 1.In operation time,the observation group was significantly shorter than the control group,the difference was statistically significant(P<0.05);in the amount of bleeding during operation,the observation group was significantly less than the control group,the difference was statistically significant(P<0.05);in the amount of blood transfusion during operation,the observation group was significantly less than the control group,the difference was statistically significant(P<0.05);in hospitalization time,the observation group was significantly shorter than the control group,the difference was statistically significant(P<0.05).2.Following up for one year,the excellent and good rate of knee joint function was 86.21%in the observation group,which was significantly higher than 62.07%in the control group,the difference was statistically significant(P<0.05).Conclusion In clinic,for patients with nonunion after intramedullary nail fixation for lower limb fracture,additional lateral plate operation is effective,so it has the value of popularization and application.
