保留距下关节截骨矫形内固定手术治疗陈旧性跟骨骨折20例分析

Analysis of 20 Cases of Old Calcaneal Fracture Treated with Subtalar Osteotomy and Orthopedic Internal Fixation

目的探讨保留距下关节截骨矫形内固定手术治疗陈旧性跟骨骨折的临床疗效。方法对20例(20足)陈旧性跟骨骨折患者采用愈伤组织切除、骨折后再复位、关节面合并、对跟骨力线矫正、恢复Bohler角和Gissane角度、骨移植填充治疗、内固定手术治疗。结果随机访问5~20个月,所有骨折患者均愈合。Maryland足部得分:优12足,良5足,可2足,差1足。术后跟骨长度,宽度,波勒角,Bohler角和Gissane角与术前明显不同,差异有统计学意义(P<0.05)。结论保留距下关节截骨矫形内固定手术治疗陈旧性跟骨骨折能取得较好效果。

Objective To investigate the clinical effect of fixed subtalar osteotomy and orthopedic internal fixation for the treatment of old calcaneal fractures.Methods Twenty patients(20 feet)with old calcaneus fractures were treated with callus resection,post-fracture reduction,articular surface commissure,correction of the calcaneus line,recovery of Bohler angle and Gissane angle,bone graft filling therapy,internal fixation surgery.treatment.Results Randomized visits for 5 to 20 months,all fracture patients healed.Maryland foot score:excellent 12 feet,good 5 feet,can be 2 feet,a difference of 1 foot.Postoperative calcaneus length,width,Poule angle,Bohler angle and Gissane angle were significantly different from those before surgery.The difference was statistically significant(P<0.05).Conclusion Preserving subtalar osteotomy and orthopedic internal fixation for the treatment of old calcaneal fractures can achieve better results.
