肱骨近端交锁髓内钉治疗肱骨近端骨折的疗效研究

Clinical Effect of Proximal Humerus Interlocking Intramedullary Nail in the Treatment of Proximal Humerus Fracture

目的:探讨应用肱骨近端交锁髓内钉治疗肱骨近端骨折的疗效。方法:本组研究对象均为博罗县人民医院2015年1月至2018年1月收治的肱骨近端骨折患者,共36例,根据不同的手术方法进行分组,交锁髓内钉组18例,应用肱骨近端交锁髓内钉手术进行治疗;锁定钢板组18例,采用锁定钢板进行固定治疗。比较两组患者手术切口大小、术中出血量、手术时间、骨折愈合时间。结果:和锁定钢板组比较,交锁髓内钉组患者手术切口明显更小,术中出血量明显更少,手术时间、骨折愈合时间明显更短,差异具有统计学意义(P <0.05)。结论:采用肱骨交锁髓内钉和锁定钢板固定治疗肱骨近端骨折,疗效均满意,但是二者对比而言,交锁髓内钉创伤更加小、操作简单,更有利于肩关节功能的恢复。

Objective To investigate the effect of proximal humeral interlocking intramedullary nail in the treatment of proximal humerus fractures.Methods The patients in this group were all patients with proximal humeral fractures from January2015 to January 2018 in Boluo people’s hospital.A total of 36 patients were divided into groups according to different surgical methods.There were 18 patients with interlocking intramedullary nails.The proximal humerus interlocking intramedullary nail was used for the treatment.The locking plate group was treated with locking plate for 18 cases.The size of the incision,the amount of intraoperative blood loss,the operation time,and the time of fracture healing were compared between the two groups.Results By compared with the locking plate group,the size of the surgical incision in the interlocking intramedullary nail group was signi?cantly smaller,the intraoperative blood loss was signi?cantly less,the operation time and fracture healing time were signi?cantly shorter,and the data were statistically signi?cant(P<0.05).Conclusion The treatment of proximal humeral fractures with iliac interlocking intramedullary nail and locking plate?xation was satisfactory,but the contrast of the interlocking intramedullary nail was smaller,the operation was simple,and it was more conducive to the recovery of shoulder function.
